PURPOSE OF SITE VISIT: Follow up Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: Corrective action is still in process. OBSERVED VIOLATIONS:
None

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 - Permit to Operate was issued on December 10, 2019 and Expires on October 08, 2053. Reviewed on Laserfiche.
02 - Facility has signage at entrance with required information.
03 - Directional signs are present but may need to add some additional signage to keep trucks and equipment off the covered part of the active cell due to traffic is exposing waste. Designate a marked single turn around area. 04 - Facility is secured by locking gates and fencing. 05 - Interior roads are constructed of all-weather materials and maintained well. 06 - Wind Blown trash was not an issue during this inspection as was noted as an observed violation on the last inspection. 07 - Corrective action is taking place on the other noted observed violations noted on the last inspection.
08 - One large area has been completed by using waddles on the top of the cell to keep the erosion to a minimum. This area has been covered and seeded. 09 - Continue the repairs to the other areas in need of this same type of repair during this period of dry weather. 10 - John College will conduct unannounced follow up inspections to ensure the remainder of the observed violations are corrected and maintained.
11 - Some wells are in need of additional information on the well ID tags. 12 - All COVID-19 CDC Recommendations were followed during this inspection.
13 - Digital pictures were taken of the areas that were noted in the observed violations on the last report and some are attached to this report.
