The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) Pumper vehicle(s) inspected:

2) The firm name, town name, phone number, and firm permit number was on both sides of the truck (on
the cab door and also on the tank). The letters were three (3) inches in height or greater. The lettering
distinguishable and not blocked from view. The lettering on both the cab door and tank meets the
requirements of 15A NCAC 13B .0835(b). Phone number 919-277-0709 was on the truck. I added the
number to the application. See pictures below.
3) The tank is made of metal and affixed to the roll off base. I requested Jim to add a bigger piece of metal
between the bed of the roll off base and the washers and nuts of the bolts attaching the tank to the bed
of the roll off base. On 04/13/2020, Jim emailed to me a picture of the bigger piece of metal that was
added as requested for all four bolts. See picture below. I also requested that the roll off base be affixed
to the frame of the truck. Jim suggested that u-bolts be placed through the metal hooks on the truck
frame and around the metal frame of the roll off base. I went with Jim’s suggestion and Jim used u-bolts
to affix the roll off base to the frame of the truck at the four corners as per 15A NCAC 13B .0835(a)(1).
Jim emailed me pictures on 04/16/2020 of the u-bolt setup for all four corners. See a picture below of
one of the u-bolt setups.
4) The valves appeared to be in working order and closed as per 15A NCAC 13B .0835(a)(2).
5) Access ports were properly sealed with lids. The lids were closed.
6) No leaks noticed as per 15A NCAC 13B .0835(a).
7) Remember to keep records of pumping and discharge events as per 15A NCAC 13B .0836. For pumping
of portable toilets, the location can be a route of toilets pumped on a given day instead of the location of
each toilet pumped on that day. I have attached an example of a truck log that you may use.
