The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) Complaint received from Division of Water Resources in the Washington Regional Office of spraying on
trees and truck emptying on the road. No evidence of septage on the road. Fields 1 and 2 have pine
trees as the permitted crop.
2) Two shallow trenches had been dug within the first row of pine trees in Field 1 along the path. Do not
dig trenches within the permitted or buffered areas. See pictures below.
3) Indentations / ruts noticed within Field 1. Be careful with applications when the site is wet. Application
over the years have made shallow indentations due to truck traffic in between the rows of trees.
However, when the site is wet, ruts are being generated from the application of septage. See picture
below. Ruts greater than 3 inches in depth are a violation of Rule .0842(a)(9).
4) Small spot dump noticed outside of the permitted boundary of Field 2. See picture below. Septage
discharged outside of the permitted boundary is a violation of Rule .0842(a)(12).
5) Required signage present.
6) Access road ok.
