S&ME, Inc., (S&ME) presents this report in accordance with the North Carolina Department of Environmental Quality
(NCDEQ)/S&ME Contract No. N17002 and S&ME Proposal No. 43-1900103B, dated October 15, 2019, as authorized by Task
Authorization No. N17002-03, dated October 15, 2019.
The scope of services included the abandonment of one monitoring well and submit a letter report. The scope of services
was conducted as proposed.
On December 17, 2019, S&ME abandoned monitoring wells MW-1A, MW-2A, MW-3A, MW-4A, MW-4, MW-5, MW-6, MW-
8, and MW-9. The well abandonment records are attached. S&ME will also submit the records to the NCDEQ Division of
Water Resources.
