FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected the vehicle in regards to issuing the firm permit. The tag number was incorrect on the firm application – the tag# is YA152358, the VIN# was correct per the firm application. The tank appeared structurally sound & securely attached. Outlet covers were on. It was raining during the inspection (could not detect any drips) but Mr. Laney stated that nothing had been in the tank yet. The lettering was correct except for the permit number as noted on the first page of this report. The town name displayed on the vehicle was Monroe (instead of Marshville as shown on the application as the office address). Mr. Laney stated that the office was actually in Monroe at the address shown on item 3 of the application (firm operators’s info). 2. Mr. Laney stated that he was going to be receiving the WWTP authorization which needs to be sent to the Raleigh Central office. I informed the Raleigh Central office that the vehicle is was in compliance on 4/23/20. No septage pump jobs shall be performed with this vehicle until after the firm permit is issued by the Raleigh Central office. Enclosed is an example truck log for your use when septage pump jobs can be performed.
