The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1. This inspection was conducted at the request of the property owner, Arthur Williams, and the facility operator,
Jamie White, for the purpose of verifying the depth of final cover on the landfill and for assessing the condition
of the side slopes and site drainage.
2. All photos included in this report were taken by Susan Heim during the inspection on April 8, 2020.
3. Access to the landfill was found to be unrestricted, with the gate removed and set to the side so that final
grading and cover work could be accomplished. Please ensure that the gate is put back into place to prevent
unauthorized access to the site.
4. Since the previous inspection, maintenance had taken place to regrade the landfill. Additional clean soil had
been applied over the entire landfill cap and tracked into the side slopes.
5. Side slopes appeared to be stable and shaped to a slope approximating a 3:1 grade.
6. The top of the landfill had been regraded and prepared for seeding. Please contact Susan Heim,
Environmental Senior Specialist, Solid Waste Section, at 336-776-9672 or via email at
susan.heim@ncdenr.gov, when a vegetative cover has been established on the landfill so that a final
inspection can be made in advance of closure of the facility.
7. 11 randomly sited probe holes had been dug by Mr. White in advance of the inspection to allow the required
12" of soil cover to be verified. All holes were inspected and measured to be in excess of 18" deep; no exposed
waste was noted in the probe holes. This serves to verify the presence of a final soil cover in excess of the
12" required by 15A NCAC 13B .0566(5).
8. Erosion control is accomplished by directing all surface water to the north end of the fill area and down into a
small sediment basin. The drainage swale on the east side of the landfill had been widened and stabilized, and
several check dams had been installed on the slope leading to the sediment basin. The north slope of the landfill
had been reshaped to facilitate the sheet flow of surface water.
9. The silt fencing that had been observed on the north slope of the landfill had been removed as requested during
the previous inspection, and no unacceptable material was found on the site.
