Volatile organic compound (VOC) detections were not reported for samples WSW-1 and WSW-5. See Table 4 for complete results. • Soil impacts greater than the soil-to-groundwater Maximum Soil Contaminant Concentrations (MSCC), but less than the residential MSCCs, were reported by Geological Resources, Inc., during installation of monitor wells in 2004. • Achieving incident closure will require the following. 1. Reduce soil impacts to levels less than the soil-to-groundwater Maximum Soil Contaminant Concentrations (MSCCs). 2. Reduce dissolved petroleum constituent concentrations in groundwater to levels less than the 2L Standard. 3.0 Scope of Services 3.1 Contract Information The scope of services for this report was performed in accordance with S&ME’s proposal number 43-1800412B, dated September 23, 2019. The scope of services was authorized by the North Carolina Department of Environmental Quality (NCDEQ’s) Task Authorization 02, dated October 3, 2019, and within contract number N17002. 3.2 Approved Scope of Services S&ME’s approved Scope of Services for this Task Authorization included the following. Sample five monitor wells (MW-1R, MW-2, MW-3, MW-4, TW-1), and measure depth to water in monitor well MW-5. Sample up to five active potable water supply wells (WSW-1, WSW-3, WSW-5, WSW-7), and prepare one field blank sample in the field. Submit samples for laboratory analysis by EPA Method 6200B, and submit the sample collected from MW- 1R for analysis by Method 6200B, Method 625, and for total Lead. Prepare this report. The scope of services were completed as proposed, with the following exceptions. There are no outside spigots for WSW-12 and the fire chief was unavailable during sampling. Water supply wells WSW-3, WSW-7 and WSW-12 were not sampled. 4.0 Site History and Characterization 4.1 UST Owner and Operator Information UST owner/operator information is provided in Table 2. 4.2 UST Information Four USTs (three 1,000-gallon gasoline and one 550-gallon kerosene) were removed from the site on August 22, 1991. An assessment and observation of the tank removal was not performed. No other USTs have observed on the site. Information about the USTs is provided in Table 1, and the locations of the former USTs and dispenser island is shown in Figure 2. Payne Oil Company operates four propane above ground storage tanks on the eastern portion of the property. 4.3 Description of the UST Release • Date discovered: January 24, 1996. • Cedar Rock Environmental collected soil samples from multiple hand auger soil borings performed during 1996 at the former UST basins and the former dispenser islands at depths ranging from 2.3 feet to 8.8 feet below ground surface (ft-bgs). Total petroleum hydrocarbons (TPH) as gasoline range organics (GRO) were detected at concentrations up to 32.5 milligrams per kilogram (mg/kg). TPH as diesel range organics (DRO) were detected at concentration up to 32,000 mg/kg. No impacted soil was excavated during or after removal of the tanks. • Several target constituents were detected in soil samples collected by Geological Resource, Inc., during assessment activities performed on March 11, 2004, at concentrations greater than the soil-to- groundwater MSCCs, but less than the residential MSCCs. 4.4 Site Characteristics • The C&H Market site is a former rural store that previously sold gasoline and kerosene. The building was razed and only remnant pavement systems remain. The former store was located in the western portion of the property, which was formerly 730 North NC 87. The property owner installed four bulk-storage propane tanks in the eastern portion of the property, currently 4357 North NC 87. A photographic log of the site and surroundings is provided as Appendix I. • Based upon the groundwater elevation map (Figure 3), groundwater flows in a radial pattern generally to the southwest and northwest. 4.5 Owners and Occupants of Adjoining/Nearby Properties and Land Use The property is located in a primarily rural residential area of Elon. No visible signs of USTs or petroleum ASTs were observed on adjoining properties. An aerial photograph of the surrounding vicinity is provided as Figure 5. Eleven active potable water supply wells are reported to be located within 1,000 feet of the source area. No water supply wells are known to remain on the subject property. S&ME interviewed Mark Johnson, owner of water supply wells WSW-4, WSW-5 and WSW-8, on July 17, 2018. Mr. Johnson stated that WSW-4 has been abandoned and that he will plans to use currently inactive WSW-8 again in the future. Information regarding water supply wells located within 1,000 feet is provided in Table 5, and their locations are shown on Figure 5. 5.0 Current Site Assessment Information 5.1 Current Liquid Levels On June 19, 2018, S&ME personnel measured depth to groundwater from the top of casing in monitor wells MW- 1R, MW-2, MW-3, MW-4, MW-5 and TW-1, using an electronic water level indicator. Depth to water measurements ranged from 14.80 feet below top of casing in monitor well MW-5 to 20.76 feet below top of casing in monitor well MW-2. Relative groundwater elevations were calculated using the depth to groundwater measurements and the relative top of casing elevations. Groundwater elevation ranged from 84.12 feet relative (ft. rel.) in monitor well MW-1R to 79.94 ft. rel. in monitor well MW-2. The depth to groundwater measurements and groundwater elevations are provided in Table 3. 5.2 Groundwater Monitoring Event On June 19, 2018, S&ME personnel collected groundwater samples from monitor wells MW-1R, MW-2, MW-4 and TW-1. Each sample was collected using a clean, disposable, polyethylene bailer. The monitor wells were purged a volume of approximately three casing volumes. The field parameters pH, specific conductivity and temperature were measured following removal of the third casing volume and recorded on the groundwater sampling forms, which are located in Appendix III. Groundwater samples were collected and transferred directly into laboratory supplied containers, placed in a cooler on ice and shipped under standard chain-of-custody record to Con-Test Analytical Laboratory (ConTest) for analysis. The groundwater samples were analyzed for VOCs by EPA method 6200B. The sample collected from MW-1R was also analyzed in the lab for semivolatile organic compounds (SVOCs) by Method 625 and for total Lead. A field blank sample was prepared using laboratory-provided water and was submitted for analysis for VOCs by EPA method 6200B. On June 19, 2018, S&ME personnel collected water samples from potable water supply wells WSW-1, WSW-3, WSW-5, WSW-7 and WSW-12, each located within 500 feet of the site. Each sampled water supply well was purged for 15 minutes. The water supply well sampling forms are included in Appendix III. The sample were collected into laboratory-supplied containers, placed in a cooler on ice, and shipped under standard chain-of- custody procedure to Con-Test for analysis. The water supply well sample was analyzed for VOCs by Method 6200B. Prior to the sampling event, S&ME requested written permission using the Request for Permission to Sample Water Supply Well form or the Permission to Sample Supply Well access agreement form, and by verbal permission via phone calls, from the property owners of each targeted water supply well. Written permission was provided by the owners of water supply wells WSW-3, WSW-5, WSW-8 and WSW-12. Verbal permission to Groundwater Monitoring Report C&H Market, Incident #17513, UST No. WS-5113 730 North NC Highway 87 Elon, Alamance County, North Carolina 27244 S&ME Project No. 4305-18-125A January 10, 2020 6 sample was provided by the owners of water supply wells WSW-1 and WSW-7 during our site visit on June 19, 2018. Copies of the permission forms are provided in Appendix III. 5.3 Summary of Groundwater Information The laboratory analytical results reported multiple target constituents were detected in monitor well MW-1R at concentration greater than the corresponding 2L Standard, including the following VOCs: Benzene, n- Butylbenzene, 1,2-Dibromoethane, Ethylbenzene, Isopropylbenzene, Naphthalene, n-Propylbenzene, Toluene, 1,2,4-Trimethylbenzene, 1,35-Trimethylbenzene and total Xylenes. SVOC concentrations greater than the 2L Standards were reported for sample MW-1R included Bis(2-ethylhexyl)phthalate, Naphthalene and 2- Methylnaphthalene. Total Lead was reported in sample MW-1R at a concentration greater than the 2L Standard. Several target constituents were detected in vertical extent monitor well TW-1 at concentrations less than their respective 2L Standards. VOCs were not reported in samples collected from monitor wells MW-2 and MW-4. VOCs were not detected in water supply well samples WSW-1, WSW-3, WSW-5, WSW-7 and WSW-12. An estimated concentration of Chloroform was detected in the field blank water sample. Groundwater analytical results are summarized on Table 4, and on Figure 4 and Figure 5. Isoconcentration contours for Benzene are shown on Figure 6. A copy of the analytical report is included in Appendix IV. On June 28, 2018, upon receipt of the analytical results, S&ME provided a copy of the report to Mr. Hassan Osman of the NCDEQ, via email, to inform him of the analytical results. 5.4 Regional and Site Geology and Hydrogeology 5.4.1 Soil and Bedrock Soils underlying the former store site are characterized as Cecil fine sandy loam. Soil boring logs (Appendix I) generally described subsurface soil as silt to a depth of 25 ft-bgs. The TW-1 boring log described weathered rock from a depth of 25 ft-bgs to 51 ft-bgs. Bedrock was not encountered by the soil borings. According to the 1985 USGS Geologic Map of North Carolina, the site is underlain by the Carolina Slate Belt, and bedrock in this area is characterized as mafic metavolcanic rock, interbedded with felsic and intermediate metavolcanic rock. 5.4.2 Hydrogeology S&ME calculated the groundwater elevations for the monitor wells gauged using the depth-to-water measurements and the top of casing elevations. The groundwater elevations were used to prepare a groundwater elevation map (Figure 3), which indicates that groundwater flows to the southwest and to the northwest from monitor well MW-1R. Groundwater occurrence was similar to conditions in 2015. Groundwater occurrence in 2015 and 2018 was generally 4 feet lower in elevation relative to measurements recorded in 2009 and 2011. The hydraulic conductivity for the site has not been determined. S&ME calculated an approximate hydraulic gradient of 0.025 feet per foot to the southwest from MW-1R and 0.0625 feet per foot to the northwest of MW-1R. A fate and transport model has not been created; therefore, the rate of contaminant migration has not been calculated. Groundwater Monitoring Report C&H Market, Incident #17513, UST No. WS-5113 730 North NC Highway 87 Elon, Alamance County, North Carolina 27244 S&ME Project No. 4305-18-125A January 10, 2020 7 5.5 Evaluation of Soil, Groundwater, Surface Water, and Free Product Assessment Information 5.5.1 Current Extent and Severity of Contamination No free product has been detected in the monitor wells at the site. No surface water assessment was performed. Dissolved phase petroleum constituent concentrations in source monitor well MW-1/MW-1R increased since the April 2015 sampling event, and the April 2015 values had increased relative to concentrations reported in 2006. The dissolved constituent plume is defined in the shallow, unconsolidated aquifer system horizontally by monitor wells MW-2 and MW-4, and has been defined historically by monitor wells MW-3 and MW-5. The plume is defined vertically in the unconsolidated aquifer system by vertical extent monitor well TW-1. Active, potable water supply wells located within 500 feet of the source area have not been impacted by VOCs. The July 2, 2018, groundwater occurrence data indicates a southwest and northwest groundwater flow direction from source monitor well MW-1R. Soil over excavation was not performed to remove soil impacted by the former UST systems. Impacts may remain beneath former UST system components at levels greater than the soil-to-groundwater MSCCs. 5.5.2 Cleanup Levels Achieved  Soil (soil-to-groundwater maximum soil contaminant concentrations) No  Groundwater (2L standard concentrations) No  Free Product - Free product has not been measured at the site. Yes 6.0 Conclusion and Recommendations Based on the current and historical assessment information for the site, the following conclusions are presented: 1. The UST Incident meets the following conditions for a High Risk classification: A. Contaminant levels in the groundwater at the site exceed the 2L Standards. B. Eleven potable drinking water wells are located within 1,000 feet of the source area. Municipal water is not available to the subject area. Based on the above, the following recommendations are presented.  Assess current soil quality conditions in the source area, in order to evaluate the feasibility of removing secondary source soil to improve groundwater quality conditions.  Monitor the stability of the dissolved constituent plume with additional sampling of groundwater monitoring wells.
