The maximum concentrations of dissolved groundwater contaminants detected in the sampled monitoring wells (MW-1, MW-2, MW-3, MW-7, MW-9, DW-1) at concentrations greater than the analytical detection limits are summarized in the table below. See Table 4 for complete results. LNAPL was not detected in remediation well RW-1 on January 14, 2020. LNAPL was not observed in the monitoring wells sampled (MW-1, MW-2, MW-3, MW-7, MW-9, DW-1). LNAPL has not previously been reported in these monitoring wells, with the exception of MW-2 from 2004 to 2011. LNAPL occurrence was less in monitoring wells MW-6R and RW-2, relative to thickness measurements recorded during July 2019, and relative to historical values. LNAPL occurrence in monitoring well MW-9 (ATC) was similar to thickness measurements recorded during July 2019 and February 2011. Monitoring Report Airport Exxon, Incident #10022, UST No. WS-3326 1200 Fairchild Road (Formerly 3751 North Liberty Street) Winston-Salem, Forsyth County, North Carolina 27105 S&ME Project No. 4305-19-128B February 17, 2020 3 • Dissolved phase petroleum constituents were reported at concentrations greater than the corresponding 2L Standards in monitoring wells MW-2, MW-3, MW-7 and DW-1. Constituent concentrations in monitoring wells MW-1, MW-2 and MW-3 were generally less relative to the January 2003 monitoring event. Volatile organic compounds (VOCs) were reported in monitoring well MW-4 and MW-5 during 2003, but these monitoring wells could not be located and are presumed to have been destroyed. Chlorinated compounds not typically associated with petroleum products were reported in vertical extent monitoring well DW-1 at concentrations less than their respective 2L Standards, with the exception of Tetrachloroethylene reported at a concentration greater than the 2L Standard. • In September 1992, two 6,000-gallon gasoline underground storage tanks (USTs), one 3,000-gallon UST, one 350-gallon heating oil UST and one 550-gallon used oil UST were removed. The gasoline USTs were located in a common basin. Upon removal of the USTs, a petroleum release was discovered at the former UST basins and the gasoline dispenser locations. Soil over excavation was performed in the areas of the former USTs and dispenser island. Total petroleum hydrocarbon (TPH) as gasoline range organic (GRO) concentrations were documented in the soil at two gasoline dispenser locations at concentrations greater than the North Carolina Department of Environment Quality (NCDEQ) action levels. • Based on the receptor survey performed during January 2003, no active potable water supply wells were noted within 1,000 feet of the site. • Achieving incident closure will require the following. 1. Remove LNAPL. 2. Implement land use restrictions for groundwater. 3.0 Scope of Services 3.1 Contract Information The scope of services for this report was performed in accordance with S&ME’s proposal number 43-1900441B dated December 13, 2019. The scope of services was authorized by the NCDEQ Task Authorization 03, dated December 13, 2019, and within contract number N17002. 3.2 Approved Scope of Services S&ME’s approved Scope of Services for this Task Authorization included the following. Professional time to search for monitoring well MW-4 and air sparge well SPG-2. Gauge monitoring wells RW-1, RW-2, MW-6 and MW-9 (ATC) for LNAPL, and collect groundwater samples from seven monitoring wells (MW-1, MW-2, MW-3, MW-4 (if located), MW-7, MW-9 and DW-1) for analysis by Method 6200B. Analyze one field blank sample by Method 6200B. Prepare this report. The scope of services was completed as proposed except for the following. • Monitoring well MW-4 was not located. Monitoring well MW-4 was not sampled. Monitoring Report Airport Exxon, Incident #10022, UST No. WS-3326 1200 Fairchild Road (Formerly 3751 North Liberty Street) Winston-Salem, Forsyth County, North Carolina 27105 S&ME Project No. 4305-19-128B February 17, 2020 4 4.0 Site History and Characterization 4.1 UST Owner and Operator Information UST owner/operator information is provided in Table 1. The owner and operator of the USTs was determined as A.W. Thomas. 4.2 UST Information Two 6,000-gallon USTs, one 3,000-gallon UST, one 350-gallon heating oil UST and one 550-gallon used oil UST operated at the site until September 1992. Information about the USTs is provided in Table 2. The locations of the former common gasoline UST basin and former gasoline dispenser islands are shown in Figure 2. 4.3 Description of the UST Release • Date discovered: September 24, 1992. • Soil over excavation was performed in the areas of the former gasoline USTs and dispenser island, as shown on the figure included in Appendix I. The initial over excavation at the former UST basin encountered groundwater at a reported depth of 14 feet below ground. The over excavation was extended based on the detection of elevated volatile organic vapor concentrations in soil samples. The total volume of excavated soil is unknown, but the reported surface area of the excavation was approximately 6,000 square feet, and the estimated excavation depth was from 5 feet to 7 feet. Risk- based soil sampling was not performed. The analytical results for TPH of confirmation soil samples collected from the excavation sidewalls near the former gasoline UST basin indicated removal of impacted soil. TPH as GRO concentrations ranging from 150 mg/kg to 2,021 mg/kg were reported in two soil samples collected from a depth of 4 feet below ground near the former dispenser island. • The June 2003 Limited Site Assessment (LSA) recommended a Low risk ranking and industrial/commercial land use. Shallow monitoring wells MW-1 through MW-5 were installed to depths ranging from 17 feet to 25 feet below ground, and vertical extent monitoring well DW-1 was installed to a depth 45 feet below ground. Depth to groundwater in the source area was approximately 12 feet below ground, and a south southeast groundwater flow direction was observed on the site at hydraulic gradients ranging from 0.016 to 0.028 feet/foot. The analytical results for soil samples collected from monitoring wells MW-1 (gasoline UST basin) and MW-3 (used oil/heating oil UST basin) in the vadose zone at depths beneath the over excavation backfill were less than the soil-to-groundwater Maximum Soil Contaminant Concentrations (MSCC). The results for soil samples collected from MW-2 (dispenser) were greater than the soil-to- groundwater MSCCs, but less than the residential MSCCs. VOCs were reported in LSA groundwater samples collected from each monitoring well at concentrations greater than the corresponding 2L Standards, but less than the Gross Contamination Levels (GCLs). Carbon-chain fraction concentrations were detected at concentrations greater than the corresponding 2L Standards in MW-1 through MW- 4. LNAPL was not observed in the monitoring wells. • LNAPL was detected in monitoring or recovery wells MW-2, MW-6, RW-1, RW-2 and MW-9 (ATC) at thicknesses ranging from 0.02 feet (MW-2) to 2.00 feet (RW-2). As summarized on Table B-8A (Appendix I), a hybrid AFVR/air sparge event performed in August 2004 removed an estimated total of 29 gallons of product as liquid. The volume of product removed as vapor is unknown. Monitoring Report Airport Exxon, Incident #10022, UST No. WS-3326 1200 Fairchild Road (Formerly 3751 North Liberty Street) Winston-Salem, Forsyth County, North Carolina 27105 S&ME Project No. 4305-19-128B February 17, 2020 5 • A July 2009 Free Product Evaluation Report documented LNAPL thicknesses recorded in MW-2, MW-6, MW-9, RW-1 and RW-2 at thickness ranging from 0.13 feet to 2.0 feet. Approximately 17 gallons of product/water was removed by hand bailing. An April 2011 Free Product Recovery System Specification Report estimated the presence of 12,648 gallons of LNAPL on the site, and recommended air sparge and soil vapor extraction to mitigate LNAPL occurrence. • S&ME visited the site on July 19, 2019, to identify monitoring wells locations, to measure the relative of casing elevation for remediation well RW-1, and to gauge wells for the presence of LNAPL. Previously identified wells were located, with the exception of MW-4, MW-5 and SPG-2. LNAPL was detected in monitoring wells MW-6, RW-1, RW-2 and MW-9 (ATC) at thicknesses ranging from 0.04 feet to 1.41 feet. 4.4 Site Characteristics • The site is a vacant, grassed field, with landscaping near the intersection of N. Liberty Street and Norfleet Street (former location of MW-5). Based on the Forsyth County GIS, the former incident parcel for 3751 N. Liberty Street was merged with other portions of the Airport Commission property. • Based upon the groundwater elevation map (Figure 3), groundwater generally flows to the south. 4.5 Owners and Occupants of Adjoining/Nearby Properties and Land Use The former incident parcel is adjoined to the north, east and south by the larger Airport Commission property. N. Liberty Street adjoins the site to the west. Surrounding properties are in industrial and commercial use. The nearest residential properties appear to be located more than 2,000 feet in each direction from the incident site. 4.6 Receptors/Potential Receptor Information Conducting a receptor survey was outside of the scope for this assessment. Previous receptor information provided by the 2003 Limited Site Assessment indicated that no water supply wells were identified within the vicinity of the site. No apparent surface water bodies are located within 500 feet of the source area. A municipal water source is available to the site and surrounding properties. 5.0 Current Site Assessment Information 5.1 Monitoring Well and Remediation Well Locations On January 14, 2020, S&ME personnel located wells not observed during the site visit performed on July 19, 2019. The locations of monitoring well MW-4 and remediation well SPG-2 were observed on historical site aerial photographs reviewed on Google Earth. The coordinates provided on Google Earth were downloaded to a GPS unit. Using the GPS unit, and triangulation of the locations of MW-4 and SPG-2 relative to other wells, S&ME identified the location of each well. Soil overlying SPG-2 was removed using hand tools. MW-4 was not found by these methods and by using a metal detector. Review of other historical aerial photographs on Google Earth indicates site grading activities occurred on portions of the site located east of monitoring wells MW-3 and MW-9. Based on these observations, MW-4 is presumed to have been destroyed. Monitoring Report Airport Exxon, Incident #10022, UST No. WS-3326 1200 Fairchild Road (Formerly 3751 North Liberty Street) Winston-Salem, Forsyth County, North Carolina 27105 S&ME Project No. 4305-19-128B February 17, 2020 6 5.2 Current Liquid Levels On January 14, 2020, S&ME personnel gauged LNAPL with an electronic oil/water interface probe in monitoring or remediation wells MW-6, RW-2 and MW-9 (ATC) at thicknesses ranging from 0.41 feet to 1.04 feet. LNAPL was not detected in remediation well RW-1. LNAPL measurements are shown on Figure 4. S&ME measured depth to groundwater from the top of casing in monitoring wells MW-1, MW-2, MW-3, MW-6, MW-7 and MW-9, using an electronic water level indicator. Depth to water measurements, in shallow wells with no detectable LNAPL, ranged from 8.95 feet below top of casing in monitoring well MW-9 to 12.43 feet below top of casing in MW-3. The depth to water in vertical extent monitoring well DW-1 was 11.63 feet below top of casing. Relative groundwater elevations were calculated using the depth to groundwater measurements, the relative top of casing elevations, and the adjusted groundwater elevations for monitoring wells exhibiting LNAPL. Groundwater elevations ranged from 88.43 feet relative (ft. rel.) in monitoring well MW-2 to 85.29 ft. rel. in monitoring well MW-9 (ATC). The liquid level measurements and groundwater elevations are provided in Table 3. 5.3 Groundwater Monitoring Event On January 14, 2020, S&ME personnel collected groundwater samples from monitoring wells MW-1, MW-2, MW- 3, MW-7, MW-9 and DW-1. Each sample was collected using a clean, disposable, polyethylene bailer. The monitoring wells were each purged approximately three casing volumes. The field parameters pH, specific conductivity and temperature were measured after the third casing volume and recorded on the groundwater sampling forms, which are located in Appendix II. During the sampling event a Field Blank sample was also prepared using laboratory supplied deionized water. Groundwater samples were collected and transferred directly into laboratory supplied containers, placed in a cooler on ice and shipped under standard chain-of-custody record to Con-Test Analytical Laboratory (ConTest) for analysis. The groundwater samples and the field blank sample were analyzed for VOCs by EPA Method 6200B. 5.4 Summary of Groundwater Information The laboratory analytical results for groundwater samples collected from monitoring wells MW-1, MW-2, MW-3, MW-7, MW-9 and DW-1, indicate that the following constituents were reported at concentrations greater than the corresponding 2L Standards. • Benzene in MW-2, MW-3, MW-7 • Xylenes, total, in MW-7 • Naphthalene in MW-2, MW-7 • Tetrachloroethylene in DW-1 No constituents were reported at concentrations greater than the corresponding GCLs. Additional constituents were reported in groundwater samples collected from MW-1, MW-2, MW-3, MW-7, MW-9 and DW-1, at concentrations or estimated concentrations less than the corresponding 2L Standards. A summary of groundwater analytical results are provided in Table 4 and on Figure 5. The laboratory analytical report and chain of custody record are provided in Appendix III. Monitoring Report Airport Exxon, Incident #10022, UST No. WS-3326 1200 Fairchild Road (Formerly 3751 North Liberty Street) Winston-Salem, Forsyth County, North Carolina 27105 S&ME Project No. 4305-19-128B February 17, 2020 7 Acetone and methylene chloride were reported in the field blank sample at estimated concentration less than the 2L Standard. 5.5 Regional and Site Geology and Hydrogeology 5.5.1 Soil and Bedrock According to the Completion Reports provided for monitoring well MW-1 through MW-5 and DW-1 (Appendix I), soil encountered at the site primarily consisted of fine sandy silt. Sandy silt was underlain by silty fine sand at depths of 8 feet to 9 feet below ground surface (ft-bgs) in soil borings MW-1, MW-2 and MW-4. Sandy silt was underlain by silty fine to course sand at a depth of 28 ft-bgs in soil boring DW-1, which was underlain by fractured, weathered rock at a depth of 44 ft-bgs. Soil boring DW-1 was terminated on an apparent quartz seam at a depth of 45.1 ft-bgs. According to the 1985 USGS Geologic Map of North Carolina, the site is underlain by biotite gneiss and schists, described inequigranular and megacrystic. Bedrock was not encountered by soil borings performed to install monitoring wells on the subject site at a depth of 45 ft-bgs. 5.5.2 Hydrogeology S&ME calculated the groundwater elevations for the monitoring wells gauged using the depth-to-water measurements and the top of casing elevations. The groundwater elevations were used to prepare a groundwater elevation map (Figure 3), which indicates that groundwater flows to the south. The hydraulic conductivity for the site has not been determined. Based on relative groundwater elevations in monitoring wells MW-2 and DW-1, a downward vertical hydraulic gradient of 0.017 feet per foot was observed on January 14, 2020. S&ME calculated the average horizontal hydraulic gradient as 0.039 feet per foot from the source area (MW-2) to downgradient monitoring well MW-7. A fate and transport model has not been created; therefore, the rate of contaminant migration has not been calculated. 5.6 Evaluation of Soil, Groundwater, Surface Water, and Free Product Assessment Information 5.6.1 Current Extent and Severity of Contamination LNAPL was measured in downgradient monitoring wells MW-6, RW-2 and MW-9 (ATC) at thicknesses ranging from 0.41 feet to 1.04 feet. LNAPL was not detected in remediation well RW-1 . The LNAPL plume is defined by monitoring or remediation wells RW-1, MW-3, MW-7, MW-8 and MW-9 (Figure 4). LNAPL occurrence was diminished relative to historical measurements, and to measurements recorded during July 2019, in wells MW-6 and RW-2, but were similar to the previous measurements recorded for MW-9 (ATC). The calculated groundwater occurrence elevations for each of these wells were similar to the July 2019 elevations. No surface water assessment was performed. Following the 1992 soil over excavation performed beneath the former dispenser island, the former gasoline UT basing, and the area located between the dispenser and USTs, risk-based analyses of confirmation soil samples resulted in the detection of VOC concentrations less than the soil-to-groundwater MSCCs, with one exception. Constituent concentrations in monitoring wells surrounding the LNAPL plume were generally lower than the previous levels reported in 2003. The dissolved phase constituent plume is not defined vertically or horizontally. However, the chlorinated compounds reported for the groundwater sample collected from monitoring wells MW-1 and MW-9, and in vertical extent monitoring well DW-1, are not typically associated with petroleum product releases. 5.6.2 Cleanup Levels Achieved  Soil (Industrial/Commercial MSCCs) Yes  Groundwater (GCL concentrations) Yes  Free Product –(0.1 feet) No 6.0 Conclusion and Recommendations Based on the current and historical assessment information for the site, the following conclusions are presented: 1. The UST Incident meets the following condition for an Intermediate Risk classification: A. LNAPL was measured in three groundwater monitoring wells. Based on the above, the following recommendation is presented. • Remove LNAPL from the subsurface. Mobile Multi-phase Extraction (MMPE) could be performed at wells currently exhibiting LNAPL, including MW-6, RW-2 and MW-9 (ATC), and at a recovery well with historic LNAPL detections (RW-1). Removal of LNAPL as vapor could be enhanced by performing a hybrid MMPE/air sparging event and utilizing existing air sparge wells SPG-1 and SPG-2. We used the GPS, metal detector and tape measure to try locating MW-4. We never found the well. We located SPG-2 and its located correctly on the map. The monitor wells were purged and sampled with disposable bailers and nylon twine. We collected pH, conductivity and temperature after three well volumes were removed. The samples were stored in a cooler on ice as they were collected.
