Mr. Ryals: WithersRavenel (WR) has completed closure UST systems at the subject site which included four
4,000‐gallon underground storage tanks (USTs) as documented within the enclosed UST Closure
Report. This report documents activities conducted at the facility in association with NCDEQ Contract
# N17005 under task authorization TA‐04, issued by the NCDEQ on April 3, 2019.
Releases were identified on the property based upon levels of Total Petroleum Hydrocarbons gasoline
range organics (TPH GRO) at concentrations exceeding the NCDEQ Action Level in 13 of the 14
sidewall samples obtained from the periphery of the petroleum UST tank pit area, as well as 10 out of
the 11 samples obtained from beneath the former petroleum distribution lines and dispensers. In
addition, free product films were observed on groundwater which entered the excavated UST tank pit
area.

1. SITE HISTORY AND CHARACTERIZATION
1.1. UST OWNER AND OPERATOR INFORMATION
The abatement action activities described in this report were conducted in association with
NCDEQ under the guidance of the NCDEQ UST Section. The UST systems are owned and
were formerly operated by Superior Realty, LLC. Three 2,000‐gallon gasoline USTs located to
the southwest of the on‐site structure, were closed in place during March 2000. The systems
documented in this report, consisting of four 4,000‐gallon gasoline USTs located to the north
of the on‐site structure were permanently closed by removal during February 2020. NCDEQ
approved removal of these USTs under NCDEQ Contract # N17005, TA‐04, issued by NCDEQ
on April 3, 2019.
The UST systems removed from the location on February 11 – 13, 2020 included four 4,000‐
gallon gasoline USTs and two gasoline pump islands with a total of three gasoline dispensers.
No information is available indicating the most recent date on which this tank systems were
operated. See Table B‐1 for additional information regarding UST systems formerly located
the site.
The site is located at the intersection of Bell Fork Road and NC Highway 1403 (Hargett
Street/Country Club Road) in Jacksonville, Onslow County, NC (Figure 1). The location is
addressed as 303 Bell Fork Road and indexed by the Onslow County GIS System as PIN #
438717027017 The property is owned by Superior Realty, LLC of Wilmington, North Carolina
and occupied by Churches Chicken as well as a small convenience store. Site photographs
taken during UST system closure activities are included in Appendix A.
1.2. HISTORICAL & SURROUNDING UST INFORMATION
The general area is shown on Figure 1 and the site is shown on Figure 2. At the facility, seven
documented petroleum underground storage tanks were in use at one time or another. Three
petroleum underground storage tanks, located in the southwestern portion of the property,
were filled in place in March 28, 2000 under the supervision of Walton Engineering. The four
USTs closed in February 2020 remained at the site but no information was available as to when
their use was discontinued.
1.3. NON-UST INFORMATION
One above‐ground storage tank (AST) was observed on the property by WR. The 550‐gallon
kerosene AST was located adjacent to the southeast corner of the building. At the time of the
site activities, this AST was out of service and empty.
1.4. DESCRIPTION OF RELEASE / OFFSITE UST RELEASES
On April 23, 1987, free product was observed by Mr. Stanley Hole and a representative of
NCDEQ (formerly NC DENR) during the excavation of the suction and vent lines of the
premium tank. Initial investigation and abatement activities were conducted under the
direction of Enviro‐Chem Environmental Services (EChem) as described in a June 1988 report.
Additional activities included work done by Walton Engineering, where three 2,000‐gal USTs
were filled in place. No excavation or removal activities were conducted by Walton
Engineering.

On May 17, 1988, EChem conducted soil borings on the site to determine the extent of a
gasoline leak at the site. Investigative activities indicated a release in the area of the dispenser
island.
On May 14, 2007, Engineering & Environmental Science Company (E2S) returned to the site
to perform further contamination evaluation at the site in the area of the northern UST basin
adjacent to the structure. One monitoring well (MW‐1) was installed at this location. During
the installation of MW‐1 on May 14, 2007, one soil sample was collected (MW‐1) from
approximately 5 ft. below ground surface. This sample was analyzed for volatile organic
compounds (VOCs) by EPA Method 8260 and for volatile petroleum hydrocarbons by the
MADEP VPH Method. The soil sample collected during the installation of MW‐1 indicated
several compounds above the Soil‐to‐Groundwater Maximum Soil Contaminant
Concentrations (MSCCs), but below Residential and Industrial/Commercial cleanup levels.
On May 14, 2007, E2S collected a groundwater sample from the newly‐installed MW‐1. This
sample was analyzed by Standard Method (SM) 6210D, and the MADEP VPH Method.
Groundwater test results indicated several compounds at levels 10 times greater than the NC
2L Standards defined by NCAC Title 15A, Subchapter 02L, Section .0202 (Water Quality
Standards for Class GA Groundwater). Results also indicated Benzene at levels above the
Gross Contamination Level (GCL).
1.5. SITE CHARACTERISTICS AND SURROUNDING PROPERTY
The site is located at the eastern quadrant of the intersection of Bell Fork Road and NC
Highway 1403 (Hargett Street/Country Club Road) in Jacksonville, Onslow County, North
Carolina (Figure 1). The site is currently owned by Superior Realty, LLC, which acquired the
property on June 25, 2007.
The portion of the property on which the four USTs were located is the northern portion of
the parcel. There were two pump islands (with a total of three dispensers) located within the
concrete pad. Around the concrete pad was asphalt surface. The one‐story structure is
located in the central portion of the site. Land uses to the northeast and southeast of the site
are residential, and commercial businesses are present to the northwest and southwest.
Based on data provided by the Onslow County GIS website, the site has an approximate
elevation of 25 ft above mean sea level (AMSL), and is relatively flat. The site is not located
within a mapped flood plain. Stormwater runoff in the vicinity of the site is channeled through
stormwater conveyances.
Electrical power is provided to the location through overhead lines, while natural gas services
used in the area are provided through buried lines. Stormwater drainage is provided through
man‐made conveyances (e.g., buried lines, drop inlets). On‐site wastewaters for properties
located in the immediate area are handled through municipal wastewater services.
The site is zoned as “Community Commercial” (CC). Surrounding properties area zoned as
either RMF‐LD (“Residential Multi‐Family – Low Density”) or RSF‐5 (“Residential Single‐Family
– 5 People”) properties.

1.6. INITIAL ABATEMENT ACTIONS AND ASSESSMENT
ACTIVITIES PERFORMED TO DATE
A Preliminary Site Evaluation and Subsurface Soil Exploration Report was completed by EChem
on June 15, 1988. Ten hand auger borings were placed and sampled to depths of 80 inches
to 85 inches. The approximate locations of these hand auger borings were in the northwest
portion of the site. As noted in the report, six boring locations possessed a petroleum odor,
which persisted from 12 inches to 80 inches below ground surface. Moreover, free product
was found in some sample locations, therefore confirming the existence of environmental
impact to the soils and groundwater.
A Phase I Limited Site Assessment was prepared by E2S on July 21, 2007. A monitoring well
(MW‐1) was installed near the western perimeter of the site and the soils and water were
sampled. Laboratory results showed benzene in the groundwater at levels that exceeded the
GCL for the Benzene and C9‐C10 Aromatic hydrocarbon concentrations exceeded the 2B
Surface Water Standards by a factor of 10, with a stream located within 500 ft. of the release.
The UST closure assessment activities conducted during February 2020 are described in
Sections 2 and 3 of this report.

2 UST CLOSURE ACTIVITIES
2.1 PREPARATIONS FOR UST CLOSURES
WR submitted the required GW/UST‐3 (Notice of Intent: UST Permanent Closure or Change‐
in‐Service) form to NCDEQ, Wilmington Regional Office on April 11, 2019. A copy of this
submittal is included as Appendix B. Eastern Environmental Management, Inc. (EEM) obtained the requisite permission to remove
the USTs from the City of Jacksonville Fire Inspections manager during closure activities on
February 11‐13, 2020. Approval for UST removals was given following testing of the oxygen
and lower explosive limits (LELs) of the atmosphere within each tank (see Appendix C for a
copy of the disposal manifest and the Fire Marshal information). Residual fluids contained in the USTs were removed by EEM using a vacuum‐equipped tanker
truck prior to removal of the USTs. A combined total of 1,596‐gal of liquids and sludge were
pumped from the USTs prior to their closure. These residuals were disposed of at EEM’s
permitted facility located in Rocky Mount, NC. See Appendix C for a copy of the disposal
manifest. A total of 235.33 tons of contaminated soils were removed from above and around the USTs
to expose the tops of the tanks as well as around the distribution lines and dispensers to allow
for their removals. Because these soils exhibited petroleum odors, and were considered
unsuitable backfill material, the soils were transported to Oak Hill Farms, a permitted soil
treatment facility in Autryville, NC for treatment and disposal. Soil disposal documentation
(manifests and certificate of disposal) is included in Appendix C. Purging of vapors and reduction of oxygen content within the tanks was accomplished through
the removal of residual fluids and vapors from the USTs using the vacuum equipped tanker
truck and addition of F‐500™, a fire, vapor, and contamination control solution manufactured
by Hazard Control Technologies, Inc. Under supervision of the City of Jacksonville Fire
Marshal, the O2/LEL levels within each tank were measured prior to their removal, and
extraction of the tanks was not performed prior to approval by the fire marshal.
2.2 UST CLOSURE PROCEDURES
EEM first removed the concrete or asphalt overlying the tanks to expose fill materials covering the
USTs. For the purposes of tank removals, those soils necessary to facilitate the removal of the
tanks were excavated from the sides and end walls of the USTs. Fill soils above the USTs also
exhibited petroleum odors reminiscent of petroleum fuels.
Each of the USTs was removed from the subsurface of the site after having been cleared for
removal by the City of Jacksonville Fire Marshal. The tanks were extracted using a trackhoe and
placed on a low‐boy trailer for transportation to the destruction/recycling location. According to
information from the site manager, the interiors of the tanks were coated in fiberglass in 2005.
Upon removal, it appeared that the outer coating on each of the tanks had deteriorated and
cracked in many places. The conditions of each tank upon removal from the subsurface are shown
below:

The USTs removed from the site on February 12, 2020 were transported to Coastal‐Carolina
Industrial Recycling in Elizabethtown, NC for destruction/recycling. The destruction/recycling
receipt for the USTs is included in Appendix D. The UST system excavation areas are illustrated
on Figure 3.
While heavily impacted soils were identified through visual observation, odors in the soils, and
readings obtained on a RAE Systems® MiniRAE 3000™ Photoionization Detector (PID), no
additional soils beyond those necessary to facilitate the UST systems closure and stabilize
excavated areas were removed from the locations. Petroleum films were observed on the surface
of groundwater which infiltrated the excavation during removal of the overburden and sidewall
soils and following removal of the USTs.
Following the UST closures, stabilization fabric was placed in the bottom of the UST excavation
and lifts of clean backfill were placed on top of the fabric. Each lift was compacted with the
trackhoe bucket prior to addition of the subsequent lift(s). A layer of crush and run fill,
approximately six to nine inches in thickness, was placed on top of the backfill and also compacted
with the trackhoe bucket. The UST excavation area measured approximately 34 ft by 34 ft and
was approximately 7.5 ft deep. The same procedure was used following the excavation of the
distribution lines and dispensers, which were adjacent to the southwest end of the UST excavation.
This area measured approximately 28 ft by 30 ft and was approximately 14‐inches deep.
2.3 TANK RESIDUALS, UST DISPOSAL
Information described in Sections 2.1 and 2.2.
2.4 INITIAL RESPONSE ACTIONS
No initial response actions, beyond the removal of liquids contained in the USTs, closure of the
tanks/product lines/dispensers, and disposal of soils associated with closure of the UST system
has occurred at the location during this time. Prior site activities are described in Section 1.6.

3 SITE INVESTIGATION
3.1 FIELD SCREENING TECHNIQUES
WR physically monitored the excavation of soil by randomly screening the soils as they were
excavated. The samples were placed in clean, Ziploc® bags and screened with the PID for the
presence of volatile vapors typical of petroleum hydrocarbons. The PID is an industry‐accepted
screening device used to detect the presence of volatile organic vapors, but is not relied upon to
determine specific levels of contamination.
3.2 SOIL SAMPLING PROCEDURES
By way of soil sampling, WR obtained sidewall samples from the periphery of the petroleum UST
system excavation area in which the four 4,000‐gal tanks were situated, and beneath the areas
where the product distribution lines and petroleum dispensers were formerly located. These
samples were submitted to Waypoint Analytical, LLC (Waypoint) for testing of Total Petroleum
Hydrocarbons (TPH) as gasoline range organics (GRO) and diesel range organics (DRO). These soil
samples were obtained such that a soil sample was collected representing its location as well as
the contents of each truckload of soil transported from the site for disposal. Samples collected for

All soil samples were collected either from the bucket of the tracked excavator or directly from
the floor of the shallow excavated area, field screened, and placed in laboratory‐prepared
containers including chemical preservatives as required by laboratory methods. WR personnel
donned clean nitrile sampling gloves for each sample location. Following collection, samples were
placed on ice in separate coolers, collected by the laboratory courier, and submitted to the
laboratory where the testing was completed.
3.3 GROUNDWATER SAMPLING PROCEDURES
No groundwater samples were obtained in association with UST system closure activities at the
facility.
3.4 QUALITY CONTROL MEASURES
Quality control measures were employed during this project and included use of clean sampling
tools, laboratory‐prepared sample bottle kits, calibrated field instruments, donning of clean sample
gloves during each sample collection and preparation effort, and a use of a third‐party laboratory
for analysis of soil samples obtained in association with completion of this project.
3.5 INVESTIGATIVE RESULTS
3.5.1 Results of Soil Sampling
Petroleum UST Excavation
Total Petroleum Hydrocarbons (GRO and DRO)
Analytical results for soil samples collected from the sidewalls of the petroleum UST excavation
(sample ID’s SS‐1 through SS‐5 and SS‐8 through SS‐16) showed levels of GRO in each sample
except SS‐8 in excess of the NCDEQ Action Level for GRO. DRO was reported in sample SS‐1,
however, the concentration did not exceed the Action Level.
Petroleum Distribution Lines & Dispenser Area
Total Petroleum Hydrocarbons (GRO and DRO)
Analytical results for soil samples collected from beneath the petroleum distribution lines and the
dispensers (sample ID’s SS‐6, SS‐7, and SS‐17 through SS‐25) showed levels of GRO in each
sample except SS‐17 in excess of the NCDEQ Action Level for GRO.
Data for the soil samples obtained in association with UST system closure activities at the site are
included in Table B‐3, and shown on Figure 3. The laboratory report and chain‐of‐custody record
associated with these samples are included in Appendix E.

4 CONCLUSIONS
WR completed the closure of UST systems consisting of four 4,000‐gallon petroleum USTs at the
site between February 11 and 14, 2020. Conclusions regarding these UST closures and the results
of laboratory testing of samples collected in association with these closures include the following:
Four 4,000‐gallon USTs were removed from the subsurface of the site on February 12, 2020. The
tanks were in marginal condition when removed from the subsurface, with minor pitting and
cracking of the exterior coating.
The fuel pumps servicing the petroleum UST system were removed from the ground on February
11, 2020, prior to closure of the USTs themselves.
A total of 1,596‐gallons of residual fluid was removed from the tanks prior to their closure. These
liquids were transported and disposed of by EEM at their facility located in Rocky Mount, NC.
A total of 235.33 tons of petroleum‐contaminated soils were removed from the site for
treatment/disposal at a facility in Autryville, NC.
Petroleum films were observed on the surface of the groundwater which filtered into the
excavation.
WR obtained 14 sidewall samples from the periphery of the petroleum UST system excavation
area in which the four 4,000‐gal tanks were situated, and 11 soil samples beneath the former
product distribution lines and petroleum dispensers. These samples were submitted to Waypoint
for testing of TPH as GRO. A single sample (SS‐1) was also analyzed for TPH‐DRO for disposal
profile purposes.
Analytical results for soil samples collected from the sidewalls of the petroleum UST excavation
(sample ID’s SS‐1 through SS‐5 and SS‐8 through SS‐16) showed levels of GRO in each sample
except SS‐8 at concentrations in excess of the NCDEQ Action Level for GRO. TPH as DRO was
reported in sample SS‐1, however, the concentration reported was beneath the NCDEQ Action
Level.
Analytical results for soil samples collected from beneath the petroleum distribution lines and the
dispensers (sample ID’s SS‐6, SS‐7, and SS‐17 through SS‐25) showed levels of GRO in each
sample except SS‐17 at concentrations in excess of the NCDEQ Action Level for GRO.
Based on visual observations made in the field and laboratory results for soil samples collected in
association with UST closures at the facility, WR concludes that releases occurred from the UST
systems closed at the facility during February 2020. WR attributes the release to the degraded
structural conditions of each UST removed from the property and from potential leaks in the
distribution piping connections. Total Petroleum Hydrocarbon GRO level were identified at levels
in excess of the NCDEQ’s Action Levels in numerous samples obtained in association with tank
closures.

Based on the presence of free product documented in previous measurements obtained from
monitoring well MW‐1 and product films observed on groundwater entering the UST area
excavation in February 2020, WR surmises that groundwater impact by petroleum contaminants
is present at concentrations likely to exceed Gross Contaminant Levels and/or NC 2L Standards.

Site History: UST/AST System & Other Release Information
Moore's Mini Mart #4
Petroleum contamination was identified in samples collected in association with a pre‐closure assessment for three USTs closed in‐
place during June 2000.
The NCDEQ‐DWM, UST Section Permits and Inspections Branch Facility Operating Permits database indicates that these USTs were registered under Facility ID #
00‐0‐0000023014 (BP Food Mart). This permit expired on March 31, 2012. The USTs covered under this permit are no longer in use, but WR could find no
information relative to their state (e.g., "in temporary closure").
Tank Installation Date from UST Section "tblAllTanks" Excel database maintained by NCDEQ.
(Incident #21746) Tanks 1‐3 listed on "tblAllTanks" as having been closed on June 26, 2000. These tanks were reported by Engineering & Environmental Science
Corp. (Phase I Limited Site Assessment ‐ July 21, 2007) to have been "closed in‐place" by Walton Engineering. WR could find no evidence of a closure report or
sampling having been completed for the closure of these USTs, however, NCDEQ correspondence relating to the incident states that petroleum contamination was
identified in samples collected in association with a pre‐closure assessment for the three USTs. NCDEQ History for the incident indicates that a Phase I LSA was
completed for the release, and that soil contamination was identified at levels exceeding the Residential MSCCs. A copy of the Phase I LSA or a report documenting
the pre‐closure assessment activities were not available for review by WR.
(Incident #3426) Petroleum contamination (gasoline) was identified in an excavation associated with the performance of maintenance on suction and vent lines
associated with the premium UST on the property. Information relating to the release does not identify which of the four remaining USTs are associated with the
release.
