Section B. Executive Summary
1. Summary of Activities Associated with Current TA
WithersRavenel, Inc. (WR) sampled the following groundwater monitoring wells for VOC analysis
by EPA Method 6200B:
• MW-308R, MW-309R, MW-0312, MW-0314, TAMW-6, TAMW-7 and TAMW-21.
2. Soil, Groundwater and Free Product Assessment Information
Soils
No soil samples were obtained in association with the monitoring activities conducted by WR
during April 2019.
Groundwater
Analytical results for groundwater samples obtained from site monitoring wells in April 2019
revealed 15A NCAC 2L Standard violations in each of the wells sampled (• MW-308R, MW-
309R, MW-0312, MW-0314, TAMW-6, TAMW-7 and TAMW-21). Benzene (MW-309R and
TAMW-7) was reported at a concentration exceeding the NC GCL for the compound. EDB (1,2-
dibromoethane) was also reported in TAMW-7 at a level above the GCL.
Based on groundwater elevation measurements from April 30, 2019 in which only a portion of
the entire monitoring well network was gauged, the direction of groundwater flow is primarily to
the north, in general conformance with previous assertions of groundwater flow.
Free Product
No free product was measured in any of the wells gauged on the site on April 30, 2019.
Historical Extents of Free Product
• MW-0313: Free phase product thicknesses ranged from a high of 1.82 ft (December
2018) to 0.11 ft (May 2011).
• MW-0314: Free phase product was detected in April 2011 at a thickness of 0.04 ft.
• TAMW-7: Free phase product was detected in December 2018 at a thickness of
0.01 ft.
3. Receptor Information
Current
There are no known impacted receptors. Water supply wells WSW-1, WSW-2, WSW-3, and
WSW-4 are located on the subject property and are all within 150 feet of the dissolved phase
plume. None of these wells were sampled in association with the current TA.

Historical Receptor Information
As part of the CSA activities, seven private water supply wells were also sampled. Analytical
results did not identify the presence of petroleum constituents at concentrations above
laboratory detection limits in any of the seven samples. In May 2011, two water supply wells
(WSW-1 and WSW-6) were sampled. Analytical results for these two samples did not identify
the presence of petroleum constituents at concentrations above the laboratory detection limits.
Two water supply wells, WSW-1 and WSW-6 were scheduled for sampling during the July 2014
sampling event. WR personnel received permission from the property owner to sample WSW-1.
However, Mr. Jasper Paramore who owns property located 2210 Hwy 33 indicated that his well
(WSW-6) was abandoned. Therefore, only WSW-1 was sampled. Analytical results for the
sample collected from WSW-1 did not identify the presence of VOCs at concentrations that
exceed their respective 2L Standards.
During the July 2017 groundwater sampling event WR spoke with the owners of WSW-3
(Rennie Whisenant) and WSW-10 (Nancy Paramore) and they informed WR that their respective
water supply wells had been abandoned and that the properties are now connected to municipal
water.
• WSW-3 is reportedly abandoned and the property connected to municipal water;
• WSW-6 is reportedly abandoned and the property connected to municipal water; and
• WSW-10 is reportedly abandoned and the property connected to municipal water.
Mr. Cox, the tenant of the subject property, informed WR that he and the property owner, Mr.
Dixon, had discussed improvements to the property such as connecting to municipal water and
repaving the parking lot.
4. Remediation Activities
WR completed a surfactant enhanced AFVR event in December 2018. Historical AFVR events
have been conducted in the past and periodic monitoring is being conducted to establish plume
stability.
5. Recommendations
Based on the results of assessment and monitoring activities conducted at the site in association
with this TA, WR recommends the following:
• Complete an updated receptor survey for the site;
• Attempt to connect the subject property to the municipal water system and abandon the
four onsite water supply wells WSW-1, WSW-2, WSW-4, and WSW-5; and
• Complete a Corrective Action Feasibility Study to develop a remediation plan for the site.

Section C. Site History and Characterization
WithersRavenel (WR) has completed a limited groundwater sampling event at the Former
Moody’s/Handy’s Country Market. The following information summarizes the site history and
characterization.
1. Description of Site Characteristics
The property is located in Chocowinity, Beaufort County, North Carolina (Figure 1). According
to the Beaufort County GIS website, http://maps.agdmaps.com/nc/beaufort/, the ±1.18-acre
property is presently owned by the Ms. Eva E. Dixon Revocable Living Trust, which obtained sole
ownership of the property on the passing of Mr. James R. Dixon on February 28, 2018. The site
is identified by the Beaufort County GIS system as parcel # 5655-40-3886.
The subject property addressed as 2211 Highway 33 contains four structures; two one-story
residential homes, a one-story brick commercial building, and a mobile home.
According to the 1991 map entitled “Generalized Geology Map of North Carolina”, this site is
located within the Yorktown and Duplin Formations of the Coastal Plain Physiographic Province.
The Yorktown Formation is typically characterized as fossiliferous sand with clay and the Duplin
Formation is typically characterized as shelly sand, sandy marl and limestone. According to the
CSA completed by The Clark Group, the shallow subsurface at the site is primarily underlain by
unconsolidated sand, silt, and clay. Generally, silty kaolinitic clay was encountered from the
ground surface to approximately 12 feet below land surface (BLS).
2. UST Information
Current site conditions and the locations of the former UST basins are illustrated on Figure 2.
The following table summarizes information regarding the USTs that were formerly utilized on
the property.

According to historical reports, UST-1 and UST-2 were located adjacent to the southeast facing
wall of the brick building while UST-3 and UST-4 were located about 100 feet to the northwest.

3. Description of Release /General Assessment History
The following summary of the assessment history of the subject property is based on WR’s
review of historical files for the site:
• The closure of four USTs by removal took place in October 2000. In addition to the
removal of the USTs, 5 cubic yards of contaminated soil and 47 total cubic yards were
disposed of offsite at a permitted facility.
• During closure of the gasoline USTs (UST-1 and UST-2), 10 soil samples were collected
from the tank pit’s walls and floor. Analytical results for six of the soil samples indicated
the presence of petroleum constituents.
• During closure of the kerosene USTs (UST-3 and UST-4), two soil samples were collected
from the tank pit’s floor. Analytical results for the two soil samples did not identify the
presence of petroleum constituents above NCDENR (currently identified as NCDEQ)
reportable limits.
• In July 2001, one shallow groundwater monitoring well (TA-10) was installed in close
proximity to the former gasoline UST pit and sampled. Analytical results for the sample
indicated the presence of benzene at a concentration above its 2L Standard.
• During early to mid-2002, Hood Richardson, PA conducted a Comprehensive Site
Assessment (CSA) on the subject property, during which 12 soil samples were collected
and analyzed for VOCs, SVOCs, and EPH and VPH. All 12 samples contained one or
more petroleum constituents at concentrations above their respective soil-to-
groundwater MSCCs. In addition, three soil samples contained one or more petroleum
constituents at concentrations above their respective Residential MSCCs.
• During the CSA, 18 groundwater monitoring wells were installed; eight permanent
shallow (Type II) wells (TAMW-1 through TAMW-7 and TAMW-21) one deep (Type III)
well (TAMW-8), and nine temporary wells (TAMW-17 through TAMW-20 and TAMW-22
through TAMW-26). Analytical results for the samples from wells TAMW-3, TAMW-5
through TAMW-7, TAMW-17, TAMW-18, TAMW-21, and TAMW-22 indicated the
presence of one or more petroleum constituents at concentrations above their respective
2L Standards.
• As part of the CSA activities, seven private water supply wells were also sampled.
Analytical results did not identify the presence of petroleum constituents at
concentrations above laboratory detection limits in any of the seven samples. In
December 2003 and January 2004, sixteen additional monitoring wells (MW-0301
through MW-0316) were installed at the site.
• In January and June 2004, free product was measured in groundwater monitoring well
MW-0313.
• Three MMPE events were conducted at the site in 2008, 2009, and 2011.

• In May 2011, Geological Resources, Inc. supervised the installation of four additional
groundwater monitoring wells (MW-0319 through MW-0322) and completed a
groundwater sampling event. Analytical results from that sampling event indicated 10
out of the 14 wells sampled (MW-0308R, MW-0309R, MW-312, MW-0314, MW-0318,
MW-0319, MW-0321, TAMW-6, TAMW-7, and TAMW-21) contained one or more
petroleum constituents at concentrations above their respective 2L Standard. In
addition, as part of the May 2011 sampling event, two water supply wells (WSW-1 and
WSW-6) were sampled. Analytical results for these two samples did not identify the
presence of petroleum constituents at concentrations above the laboratory detection
limits.
• In July 2014, WR personnel mobilized to the former Moody’s Country Market to sample
15 groundwater monitoring wells and two water supply wells. Measurable free phase
product (FPP) was observed in MW-0313 at a thickness of 0.67 feet and was therefore
not sampled. The analytical results for the groundwater samples from MW-0312, MW-
0314, MW-0320, MW-0321, MW-308R, MW-309R, TAMW-7, TAMW-1, TAMW-21,
and TAMW-6 identified concentrations of one or more petroleum constituents that are
above their 2L Standard. Additionally, the samples from MW-0309R and TAMW-7
contained EDB and benzene at concentrations that exceed their respective GCLs for
groundwater. WR personnel received permission from the property owner to sample
WSW-1. However, Mr. Jasper Paramore who owns property located 2210 Hwy 33
indicated that his well (WSW-6) was abandoned. Analytical results for the sample from
WSW-1 did not identify the presence of VOCs at concentrations above the laboratory
detection limits.
o WR personnel interviewed Mr. Erick Jennings, the Water Systems Manager for
Beaufort County Water Department, regarding access to municipal water to the
area. He stated that a water line runs adjacent to Highway 33 and Bear Creek
Road.
• In December 2018, WR personnel performed a surfactant enhanced AFVR event
including the injection and recovery of Enviroclean® surfactant. Prior to the injection
and recovery of the surfactant, WR obtained Notice of Intent permit number WI0700479
from the Water Quality Regional Operations Section of NCDEQ. Free product was
measured in wells MW-0313 and TAMW-7. Ultimately, the Enviroclean® surfactant was
injected into each of these wells. At the completion of the injection event, monitoring
well MW-313 had received approximately 325 gallons of 2% surfactant and TAMW-7
had received approximately 175 gallons of 2% surfactant. Ten days following the
injection, an AFVR event was completed in which 2,275 gallons of petroleum contact
water were recovered from MW-313 and TAMW-7. Gauging of these wells following
the completion of the AFVR event did not reveal the presence of measurable free
product.

4. Information on Receptors/Potential Receptors
A receptor survey was conducted as part of the CSA activities. According to the report, the
subject property and surrounding properties on NC Highway 33 West are supplied water by
domestic water supply wells. The survey identified the presence of 34 supply wells within 1,500
feet of the release area. According to The Clark Group CSA, the closest surface water to the site
was a man-made pond approximately 75 feet from the source area.
Information obtained from property occupants / owners by WR during the July 2014 and July
2017 sampling events indicate that WSW-3, WSW-6, and WSW-10 have been abandoned and
the properties have been connected to municipal water.

Section D. Presentation of Current/Comparison to Historical Assessment
Information
On April 30, 2019, WR personnel mobilized to the subject to perform sampling activities in
accordance with task authorization TA-03. The current monitoring well layout is illustrated in
Figure 2 and well construction information is provided in Table B-7.
1. Groundwater Elevations/Free Product Level Measurements
Prior to purging the monitoring wells targeted for sampling in April 2019, WR personnel
obtained depth to groundwater measurements. The depth-to-water measurements ranged from
2.92 feet below the top of casing (ft-BTOC) in monitoring well MW-0309R to 6.38 ft-BTOC in
monitoring well MW-0314. No measurable free product was noted in any of the wells gauged
on April 30, 2019.
Based on groundwater elevation measurements from April 30, 2019 in which only a portion of
the entire monitoring well network was gauged, the direction of groundwater flow is primarily to
the north, in general conformance with previous assertions of groundwater flow. Gradient as
modeled based on April 2019 data is depicted on Figure 3.
2. Groundwater Sampling and Analytical Results
Monitoring Well Sampling
WR personnel obtained groundwater samples from the following monitoring wells:
• MW-308R, MW-309R, MW-0312, MW-0314, TAMW-6, TAMW-7 and TAMW-21.
After measuring depth-to-water, each sampled well was purged with a virgin polyethylene bailer.
Following purging and adequate recharge of groundwater within each well, samples were
decanted directly from the bailer(s) into laboratory prepared sample containers holding
appropriate preservative as required. Groundwater samples were submitted to Prism
Laboratories, Inc. (Prism) for analysis of VOCs by SM6200B. The laboratory analytical report and
chain-of-custody record associated with these samples are included in Appendix A. Copies of
field notes are included in Appendix B. Photographs of wells sampled in April 2019 are included
in Appendix C.
Laboratory Results
Analytical results for groundwater samples obtained from site monitoring wells in April 2019
revealed 15A NCAC 2L Standard violations in the following wells:
• MW-308R, MW-309R, MW-0312, MW-0314, TAMW-6, TAMW-7 and TAMW-21.
Analytical results for groundwater samples obtained from site monitoring wells in April 2019
revealed GCL Standard violations in the following wells:
• MW-309R (benzene) and TAMW-7 (benzene, EDB)
The groundwater analytical results are summarized in Table B-4 and shown on Figure 4.

Section E. Conclusions and Recommendations
The following conclusions can be made from the April 30, 2019 limited groundwater sampling
event:
• No free product was measured in any of the monitoring wells gauged or sampled in
association with this limited groundwater monitoring event.
• One or more targeted compounds were reported in samples from each of the wells
sampled in association with this event (MW-308R, MW-309R, MW-0312, MW-0314,
TAMW-6, TAMW-7 and TAMW-21) at concentrations exceeding their respective 2L
Standards.
• Benzene (MW-0309R and TAMW-7) and EDB (TAMW-7) were reported at levels
exceeding their respective GCL concentrations.
• According to the property owners, WSW-3, WSW-6 and WSW-10 have been abandoned
are connected to municipal water; The closure of these wells means that there are no
directly downgradient active water supply wells. Mr. Cox informed WR that he and Mr.
Dixon are open to connecting the site to municipal water and abandoning the onsite
supply wells.
Based on the information gathered during the recent groundwater and water supply well
sampling event, WR recommends the following:
• Complete an updated receptor survey for the site;
• Attempt to connect the subject property to the municipal water system and abandon the
four onsite water supply wells WSW-1, WSW-2, WSW-4, and WSW-5; and
• Complete a Corrective Action Feasibility Study to develop a remediation plan for the site.
