Mr. Ryals:
WithersRavenel, Inc. (WR) mobilized to the above‐referenced site on February 21, 2020 to complete
activities associated with Task Authorization 1, which was approved by the UST Section on February 7,
2020 under NCDEQ Contract #N17005.
WR was tasked with collecting groundwater samples from site monitoring wells MW‐1, MW‐2, MW‐3,
MW‐4, and DW‐1. WR was unable to locate monitoring wells MW‐1, MW‐2, MW‐4, and DW‐1.
Monitoring well MW‐3 was found to be abandoned with concrete.
While onsite, WR observed that the onsite underground storage tank (UST) was not in‐use and not
abandoned. According to the NCDEQ UST Database, the onsite UST is a 12,000‐gallon UST divided into
an 8,000‐gallon compartment and a 4,000‐gallon compartment.
WR has included a photographic record of the February 21, 2020 site visit as an attachment.
WR recommends closing the onsite UST by removal.
