EXECUTIVE SUMMARY The purpose of this report is to evaluate the current status of groundwater quality at the Mustang #22 site located at 2111 Connelly Springs Road in Lenoir, Caldwell County, North Carolina in an area primarily developed as rural residential. The site contains a currently active automotive repair shop. A release at the site was confirmed in June 2001 when gasoline-range petroleum hydrocarbons were detected in soils within the former UST basin at concentrations above the regulatory action level. The former on-site USTs consisted of a 3,000-gallon gasoline tank, a 7,500-gallon gasoline tank and a 10,000-gallon gasoline tank. Information concerning the closure of the USTs is unknown. Terracon mobilized to the site on October 23, 2019 to conduct groundwater sampling activities at on-site monitoring wells (MW-1 through MW-4) and replace the O-sox insert in well MW-1. Initially, Terracon gauged the groundwater levels in the wells. Depth to groundwater measurements ranged from approximately 19.11 feet below top of casing (btoc) in well MW-3 to 23.35 feet btoc in well MW- 1. Based on groundwater elevations and topography, the estimated groundwater flow direction is to the northwest. Prior to sampling, each of the wells was purged a minimum of three well volumes. The groundwater samples were placed in laboratory-supplied glassware, labeled and stored in an ice-packed cooler. The samples, along with chain-of-custody documentation, were delivered to Pace Analytical Services, Inc. in Huntersville, North Carolina for analysis of VOCs by Standard Method 6200B including MTBE, IPE, and EDB. Laboratory results indicated benzene [332 micrograms per liter (µg/L)], ethylbenzene [1,260 µg/L], naphthalene [419 µg/L], toluene [2,840 µg/L], 1,2,4-trimethylbenzene [1,240 µg/L], and total xylenes [5,390 µg/L] were detected above their respective 15A NCAC 2L groundwater standards (2L standards) in well MW-1. Other petroleum constituents were detected in monitoring well MW-1 at concentrations below their respective 2L standards. No petroleum constituents were identified in monitoring wells MW-2, MW-3 and MW-4 above the method detection limits (MDLs). Methylene chloride [1.5J µg/L], a common laboratory contaminant, was detected in the equipment blank. Based on information obtained during the completion of these assessment activities, Terracon provides the following conclusions and findings. Free product was not encountered at the site and none of the detected constituents exceeded their respective GCLs. Benzene, ethylbenzene, naphthalene, toluene, 1,2,4-trimethylbenzene, and total xylenes were detected above their respective 2L standards in well MW-1. Well MW-1 is located adjacent and down-gradient of the former UST basin. Constituent concentration trends are generally decreasing in well MW-1. Isopropylbenzene and styrene were detected in monitoring well MW-1 at concentrations below their respective 2L standards. No petroleum constituents were identified in monitoring wells MW-2, MW-3 and MW-4 at concentrations above the MDLs. The estimated groundwater flow direction at the site is to the northwest. Based on the findings of our assessment activities, the following comments and recommendations are provided for the site. On-site groundwater monitoring wells (MW-1 through MW-4) should be sampled semi-annually to evaluate the status of impact by petroleum constituents. Terracon recommends continued replacement of O-Sox inserts into well MW-1 on a quarterly basis to reduce petroleum constituent concentrations. C. SITE HISTORY AND CHARACTERIZATION The purpose of this report is to evaluate the current status of groundwater quality at the Mustang #22 site located at 2111 Connelly Springs Road in Lenoir, Caldwell County, North Carolina in an area primarily developed as rural residential. Terracon completed this report to summarize the findings of assessment activities performed on October 23, 2019. The site is occupied by a currently active automotive repair shop. This Subsequent Monitoring Report was prepared for the site in response to a request by NCDEQ Incident Manager, Mr. Scott Ryals with the NCDEQ Division of Waste Management, State Lead Program. The Subsequent Monitoring Report was prepared in general accordance with the 15A NCAC 2L.0115 regulations and the NCDEQ Underground Storage Tank (UST) Section Guidelines for Assessment and Corrective Action for UST Releases, dated July 15, 2008, revised December 2013, under the supervision of Mr. Christopher L. C.1 Site Setting The Mustang #22 site located at 2111 Connelly Springs Road in Lenoir, North Carolina is comprised of approximately 0.37 acres. The site contains an approximate 1,300 square foot building currently occupied by an automotive repair shop. The site is depicted on Figures 1 and 2. The site is bound to the southwest by Connelly Springs Road and to the north by Greenbrier Lane. Commercial and residential properties are located to the northwest and southeast of the site along Connelly Springs Road while wooded areas and residences are located to the east and west of the site. C.2 Release Summary According to previous assessment reports, the site formerly operated a 3,000-gallon gasoline tank, a 7,500-gallon gasoline tank, and a 10,000-gallon gasoline UST. The tanks were reportedly removed prior to June 2001. A release at the site was confirmed in June 2001 when gasoline-range petroleum hydrocarbons were detected in soils obtained from the former UST basin at concentrations above the regulatory action level. C.3 Previous Investigations Geological Resources, Inc. (GRI), conducted a receptor survey and soil assessment activities at the site in July 2004 to evaluate impacts to on-site soils and groundwater and identify potential receptors near the site as a result of the release detected in June 2001. The receptor survey identified four water supply wells (WSW-1 through WSW-4) within a 500-foot radius of the source area. Well WSW-1 is located on the site approximately 100 feet from the source area, while the other three water supply wells are located on off-site properties approximately 280, 300, and 475 feet from the source area. At the time of the survey in 2004, three of the four water supply wells were in use, and well WSW-4 was reportedly abandoned. Municipal water was reportedly available to the site area. GRI attempted to install a monitoring well within the former UST basin; however, rock was encountered at a depth of approximately 12 feet below ground surface (bgs). GRI collected a soil sample from the boring, and advanced twelve additional soil borings (SB-1 through SB-12) near the former UST basin. Petroleum constituent concentrations exceeded the Soil-to-Water Maximum Soil Contaminant Concentrations (MSCCs) in soil borings SB-1, SB-2, SB-6, SB-8 and SB-12. None of the detected constituents exceeded the Residential MSCCs. S&ME, Inc. (S&ME) completed a Limited Site Assessment (LSA) in November 2009. According to the report, S&ME mobilized to the site on June 5, 2009 to collect soil samples and a groundwater sample from monitoring well MW-1. Analytical results of the soil samples indicated petroleum constituents at concentrations exceeding the Soil-to-Water MSCCs, but below the Residential Soil Cleanup Levels. S&ME installed monitoring well MW-1 to a depth of approximately 40 feet bgs. Based on the analytical results from well MW-1 and the presence of water supply wells within 1,000 feet of the source area, three additional monitoring wells (MW-2, MW-3 and MW-4) were installed on the site. Monitoring wells MW-2 and MW-3 were installed topographically downgradient of the approximate location of the former UST basin, and monitoring well MW-4 was installed topographically up- gradient of the former UST basin. Analytical results of groundwater samples collected from the on- site wells detected various petroleum constituents in wells MW-1 and MW-2 at concentrations exceeding their respective 2L standards, while none of the target analytes were detected above laboratory reporting limits in wells MW-3 and MW-4. As part of the LSA, S&ME sampled three water supply wells on July 11, 2009. At the time of the groundwater sampling event, on-site well WSW-1 was not operational but was sampled using a disposable polyethylene bailer. The groundwater samples from wells WSW-2 and WSW-3 were collected from spigots located at the on-site residences. The water supply well samples were analyzed for VOCs by EPA Method 6200B. No target analytes were detected in wells WSW-1 or WSW-2 but MTBE was detected in well WSW-3 below its 2L standard. Terracon personnel mobilized to the site on March 19, 2015 to collect groundwater samples from on-site monitoring wells MW-1 through MW-4 to evaluate current groundwater conditions. Benzene was detected at a concentration above its 2L groundwater standard in monitoring wells MW-1 (873 µg/L) [labeled as well MW-4 in the laboratory report] and MW-2 (13.8 µg/L). In addition to benzene, 1,2-dibromoethane (EDB), ethylbenzene, isopropylbenzene, methylene chloride, naphthalene, n- propylbenzene, toluene, 1,2,4-trimethylbenzene, 1,3,5-trimethylbenzene, and xylenes were also detected at concentrations above their respective 2L standards in well MW-1. Other constituents detected in the wells were below their respective 2L standards. Terracon mobilized to the site on April 2, 2018 to conduct groundwater sampling activities for on-site monitoring wells MW-1 through MW-4. Benzene, ethylbenzene, ispropylbenzene, naphthalene, toluene, 1,2,4-trimethylbenzene, and total xylenes were detected above their respective 2L standards in monitoring well MW-1. Chloroform, MTBE and styrene were detected in well MW-1 at concentrations below their respective 2L standards. No petroleum constituents were identified in monitoring wells MW- 2, MW-3 and MW-4 at concentrations above the MDLs. Terracon mobilized to the site on October 24, 2018 to conduct groundwater sampling activities for on- site monitoring wells (MW-1 through MW-4). Depth to groundwater measurements in the wells ranged from approximately 15.22 feet (well MW-3) to 21.53 feet (well MW-1) below top of casing. Free product was not encountered at the site and none of the detected constituents exceeded their respective GCLs. Benzene, ethylbenzene, isopropylbenzene, naphthalene, toluene, 1,2,4-trimethylbenzene, and total xylenes were detected above their respective 2L standards in well MW-1. No petroleum constituents were identified in monitoring wells MW-2 and MW- 4 at concentrations above the MDLs. In addition to the groundwater sampling event, Terracon also performed a receptor survey to update the status of water supply wells within 1,500 feet of the site (discussed below). Terracon mobilized to the site on May 17, 2019 to conduct groundwater sampling activities at on-site monitoring wells (MW-1 through MW-4) and replace the O-sox insert in well MW-1. Depth to groundwater measurements ranged from approximately 11.86 feet (MW-3) to 15.75 feet (MW-2). Laboratory results indicated benzene, ethylbenzene, isopropylbenzene, naphthalene, n- propylbenzene, toluene, 1,2,4-trimethylbenzene, 1,3,5-trimethylbenzene and total xylenes were detected above their respective 2L standards in well MW-1. Other petroleum constituents were detected in monitoring wells MW-1 and MW-3 at concentrations below their respective 2L standards. No petroleum constituents were identified in monitoring wells MW-2 and MW-4 above the MDLs. Free product was not encountered at the site and none of the detected constituents were above their GCLs. C.4 Water Supply Wells S&ME reported six water supply wells were identified within 1,000 feet of the source area in 2009. During our updated receptor survey (2018), Terracon identified four water supply wells within 1,000 feet of the source area. Terracon also submitted letters to certain addresses located within 1,500 feet of the source area to update the receptor survey and confirm the operational status of the identified wells. Of the eight letters sent to nearby properties, Terracon received four responses. Based on the results, well WSW-4 (580 feet northeast) is inactive and the property is connected to municipal water. According to the reply from the property owner at 1421 Wimberly Drive, no water supply well is located at the property. The remaining two responses were for properties greater than 1,000 feet from the source area. Water supply well information is summarized in Table 1. The approximate locations of the identified water supply wells are shown on Figure 3. C.5 Surface Water According to the USGS topographic map, an unnamed tributary to Little Gunpowder Creek is located about 400 feet northeast of the site and Stafford Creek is located approximately 450 feet northwest of the site. Surface waters are depicted on Figure 1. C.6 Land Use Information The site is bound to the southwest by Connelly Springs Road and to the north by Greenbrier Lane. Commercial and residential properties are located to the northwest and southeast of the site along Connelly Springs Road. Wooded areas and residences are located to the east and west of the site. ASSESSMENT INFORMATION D.1 Site Geology and Hydrogeology The following resources were reviewed to evaluate site-specific geological and hydrogeological characteristics. Drexel Quadrangle, North Carolina, USGS 7.5-minute series topographic maps, dated 1993 (Figure 1). Geologic Map of North Carolina, dated 1985. The Geology of the Carolinas, Carolina Geological Society Fiftieth Anniversary Volume, edited by J. Wright Horton, Jr., and Victor A. Zullo, The University of Tennessee Press, dated 1991. Soil Survey of Caldwell County, North Carolina, US Department of Agriculture, Natural Resources Conservation Service, Web Soil Survey via the Internet, http://websoilsurvey.nrcs.usda.gov/app/. The site is located within the Inner Piedmont Belt of the Piedmont Physiographic Province. The Piedmont Physiographic Province generally consists of low to medium elevation, rounded hills and long rolling northeast to southwest trending ridges with incised creeks and river channels. Rocks underlying the site are characterized as mica schist. In-place chemical and physical weathering of parent rocks typically form residual soils in the Piedmont of North Carolina. Weathering is facilitated by the presence of fractures, joints, and less resistant minerals in the rock. In areas not altered by erosion, alluvial deposition or the activities of man, the typical residual soil profile consists of clay-rich unconsolidated materials at the surface, transitioning to sandy silts and silty sands at depth. Typically, the boundary between soil and rock is not sharply defined. This transitional zone is termed “partially weathered rock” or saprolite. Information obtained from the Web Soil Survey for Caldwell County, depicts soils at the site as Woolwine-Fairview-Urban Land Complex (WuB). In the Piedmont, groundwater generally occurs in pore spaces within soils and in the structural features present in the underlying rock (i.e. joints, fractures, and faults). Recharge to the water table is primarily by precipitation infiltrating the surficial soils and percolating downward under the influence of gravity to the water table (vadose zone). Typically, the water table is not a level surface, but a subdued reflection of the land surface. The depth to the water table is not consistent and is dependent upon several factors that include the amount of rainfall, permeability of the soil, the extent of fracturing in the underlying rock and the influences of groundwater pumping. Groundwater typically flows in directions parallel to the ground surface and under the influence of gravity, migrates to discharge points such as surface water features, the toes of slopes or natural springs. Fluctuations in the depth of the water table can be expected depending on variations in precipitation, surface water run-off and other factors not evident at the time of our subsurface exploration. Based on an interpreted groundwater flow direction from historical data, groundwater is expected to flow to the northwest towards tributaries of Stafford Creek. D.2 Sampling Activities D.2.1 Free Product Evaluation Free product was not encountered at the site. D.2.2 Groundwater Sampling Activities Terracon mobilized to the site on October 23, 2019 to conduct groundwater sampling activities at on-site monitoring wells (MW-1 through MW-4) and replace the O-sox insert in well MW-1. Initially, Terracon gauged the groundwater levels in the wells. Depth to groundwater measurements ranged from approximately 19.11 feet below top of casing (btoc) in well MW-3 to 23.35 feet btoc in well MW- 1. Based on groundwater elevations, the estimated groundwater flow direction is to the northwest. Current and historical groundwater elevation measurements are provided in Table 3 and a groundwater elevation map depicting groundwater flow direction is shown on Figure 4. Prior to sampling, each of the wells was purged a minimum of three well volumes. The groundwater samples were placed in laboratory-supplied glassware, labeled and stored in an ice-packed cooler. The samples, along with chain-of-custody documentation, were delivered to Pace Analytical Services, Inc. in Huntersville, North Carolina for analysis of VOCs by Standard Method 6200B, including MTBE, IPE, and EDB. D.2.3 Analytical Results Laboratory results benzene [332 µg/L], ethylbenzene [1,260 µg/L], naphthalene [419 µg/L], toluene [2,840 µg/L], 1,2,4-trimethylbenzene [1,240 µg/L], and total xylenes [5,390 µg/L] were detected above their respective 2L standards in well MW-1. Isopropylbenzene [63.6 µg/L] and styrene [26.5 µg/L] were detected in monitoring well MW-1 at concentrations below their respective 2L standards. No petroleum constituents were identified in monitoring wells MW-2, MW-3 and MW-4 above the MDLs. The groundwater analytical results are summarized in Table 2 and are depicted on Figure 5. A copy of the laboratory analytical report is provided in Appendix A. E. CONCLUSIONS AND RECOMMENDATIONS Based on information obtained during the completion of these assessment activities, Terracon provides the following conclusions and findings.
