or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle, tag#YA155890, VIN#3ALHCYFE8LDMC9989, 4,000 gallon capacity. The tank
appears structurally sound & securely attached. The outlet covers were on and no drip/leaks were noted. The
lettering is correct on both sides of the vehicle per the info on the firm application. I have informed the Raleigh
Central Office that the 2020 firm permit should have the firm name as “Enviro Choice” – that is the name
displayed on the vehicles inspected this year. I believe this is the last vehicle under this firm permit to be
inspected. Please contact me for an inspection in the future if another vehicle is added to this firm permit.
