Dispenser: includes the dispenser and the equipment used to connect the dispenser to the piping (e.g., a release from a suction pump or from components located above the shear valve) Submersible Turbine Pump (STP) Area includes the submersible turbine pump head (typically located in the tank sump), the line leak detector, and the piping that connects the submersible turbine pump to the tank Delivery Problem: identifies releases that occurred during product delivery to the tank. (Typical causes associated with this source are spills and overfills.) Other: serves as the option to use when the release source is known but does not fit into one of the preceding categories (e.g., for releases from vent lines, vapor recovery lines, and fill lines) Unknown: identifies releases for which the source has not been determined Definitions of Causes Spill: use this cause when a spill occurs (e.g., when the delivery hose is disconnected from the tank fill pipe or when the nozzle is removed from the dispenser) Overfill: use when an overfill occurs (e.g., overfills may occur from the fill pipe at the tank or when the nozzle fails to shut off at the dispenser) Physical or Mechanical Damage: use for all types of physical or mechanical damage, except corrosion (e.g., puncture of tank or piping, loose fittings, broken components, and components that have changed dimension) Corrosion: use when a metal tank, piping, or other component has a release due to corrosion (e.g., for steel, corrosion takes the form of rust) Installation Problem: use when the problem is determined to have occurred specifically because the UST system was not installed properly Other: use this option when the cause is known but does not fit into one of the preceding categories (e.g., putting regulated substances into monitoring wells) Unknown: use when the cause has not been determined Source: Base Map provided by Mecklenburg County GIS Location Map //Ad& Llppard Property 5700 Closeburn Road SUMMIT Charlotte, NC f I, Saussy Burbank 2550 West Tyvola Road Suite 100 Charlotte, NC 28217 ilk Checked By\Date: MZ/12/17 Scale Not to Scale Figure 1 Page 1 of 1 Table 1.0: Soil Sample Summary Table All Values in mg/kg CAS Analyte 1330-20-7 Xylenes (total) 92-52-4 1,1 - Biphenyl 86-73-7 Fluorene 91-57-6 2-Methylnaphthalene 91-20-3 Naphthalene 67580 Phenanthrene 129-00-0 Pyrene N/A Aliphatics, C5-C8 N/A Aliphatics, C9-C18 N/A Aliphatics, C19-06 N/A Aromatics, C9-C22 MSCCs Soil to Water Maximum MSCCs Residential Cleanup T-1 North 1/5/2018 8' bgs Result RL T-1 South 1/5/2018 8' bgs Result RL 4.60E+00 3.13E+03 3.8 3.9 NA NA 8.4 8.1 8.7 8.1 4.70E+01 6.20E+02 12.0 1.6 13.0 1.6 3.60E+00 6.30E+01 110.0 1.6 110.0 1.6 1.60E-01 3.13E+02 16.0 1.6 16.0 1.6 5.60E+01 4.69E+02 30.0 1.6 32.0 1.6 2.70E+02 4.69E+02 5.2 1.6 5.7 1.6 6.80E+01 9.39E+02 55.0 46.0 36.0 46.0 5.40E+02 1.50E+03 4900.0 460.0 5100.0 460.0 NA 3.10E+04 980.0 610.0 1000.0 610.0 3.10E+01 4.69E+02 4600.0 61.0 4040.0 61.0 ND = Below P ND = Below Method Detection Limit BOLD = Concentration Above Reference Values J = Estimated Concentration 5700 C/oseburn Road - Lippard January 22, 2018 Charlotte, NC SUMMIT Project No: 1820.75 1 /23/2018 Property Detail Print View #"V. Lot Number: TA09026 This Report of Analysis contains the analytical result(s) for the sample(s) listed on the Sample Summary following this Case Narrative. The sample receiving date is documented in the header information associated with each sample. All results listed in this report relate only to the samples that are contained within this report. Sample receipt, sample analysis, and data review have been performed in accordance with the most current approved NELAC standards, the Shealy Environmental Services, Inc. ("Shealy") Quality Assurance Management Plan (QAMP), standard operating procedures (SOPs), and Shealy policies. Any exceptions to the NELAC standards, the QAMP, SOPs or policies are qualified on the results page or discussed below. Where applicable, all soil sample analysis are reported on a dry weight basis unless flagged with a "W" qualifier If you have any questions regarding this report please contact the Shealy Project Manager listed on the cover page. VPH Surrogate recovery for the following samples was outside control limits: TA09026-001, TA09026-002. Evidence of matrix interference is present; therefore, re -extraction and/or re -analysis was not performed. SVOCs by GC/MS Due to the large number of spiked analytes, there is a high probability that one or more analytes will recover outside acceptance limits. The laboratory's SOP allows for 10% of analytes to recover marginally outside criteria. The following analytes recovered marginally outside LCS/LCSD criteria: 3,3'-Dichlorobenzidine and 2-methyl phenol. The laboratory control sample (LCS) for analytical batch 61529 exceeded acceptance criteria for the following analytes: pyridine. These analytes were biased high and were not detected in the samples affected: TA09026-001, TA09026-002. EPH Surrogate recovery for the following samples was outside control limits: TA09026-001, TA09026-002. Evidence of matrix interference is present; therefore, re -extraction and/or re -analysis was not performed. VOCs by GC/MS The following sample was diluted due to the nature of the sample matrix: TA09026-001, TA09026-002. The LOQ has been elevated to reflect the dilution.
