Site closed in accordance with the Closure letter dated July 9, 1992, and NC Solid Waste Management Rule 15A NCAC 13B .0510. PURPOSE OF SITE VISIT: Comprehensive Facility Inspection. STATUS OF PAST NOTED VIOLATIONS: 1. 15A NCAC 13B .0503(2)(a)(ii) — Corrective actions ongoing for landfill gas (LFG) exceedances at the facility boundary. As required, corrective actions for LFG exceedances at the facility boundary are ongoing. Since the previous inspection, the facility submitted a Landfill Gas Corrective Action Plan (FID 1226717), dated June 28, 2018, which proposed the installation of three passive methane gas cutoff trenches and two new passive LFG vents. This plan was approved by the Section in a letter to the facility dated July 12, 2018 (FID 1226720). The facility submitted a Gas Vent Installation Report (FID 1377056), dated November 15, 2019, which confirmed the installation of gas vents #GV-4 and #GV-5 at the site. Additionally, as required, the facility has conducted quarterly LFG monitoring. Since the previous inspection, quarterly LFG environmental monitoring reports for the facility dated 2/14/20, 11/13/19, 9/4/19, 5/16/19, 2/15/19, Page 1 of 5 FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management NORTH CAROLINA Solid Waste Section 12/13/18, 10/5/18, 5/21/18, 3/9/18, 12/20/17, 10/2/17, and 6/30/17 were available for review. Please continue to work with Ms. Jaclynne Drummond, Section Hydrogeologist, regarding ongoing LFG corrective actions at the facility. 2. 15A NCAC 13B .0510(c) — Resolved. As required, the facility submitted a revised Post -Closure Care Plan (DIN 28399) to the Section in July 2017, which detailed ongoing monitoring and remediation activities at the site. This plan was approved by the Section in a letter to the facility dated September 5, 2017 (DIN 28434). OBSERVED VIOLATIONS: None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS * For a site history and background, please see the March 8, 2017, facility compliance inspection report. 1. On -site March 11, 2020, to inspect the closed municipal solid waste (MSW) landfill. 2. Post -inspection, additional facility research conducted which concluded on March 23, 2020. 3. Access to the site is located off Rhodhiss Road (S.R. 1611) and is controlled by a lockable gate. 4. Access roads at the site were maintained. Continue to maintain site access roads as necessary. 5. Edge -of -waste (EOW) markers were observed around the closed MSW landfill and appeared to be maintained. Continue to maintain EOW markers at the facility as necessary. 6. No apparent leachate breakouts or seeps were observed during this inspection. 7. Semi-annual groundwater and surface water monitoring reports for the facility dated March 4, 2020 (for Fall 2019 sampling event) and September 3, 2019 (for Spring 2019 sampling event) were available for review. As required, continue to monitor all groundwater and surface water sampling locations at the facility and submit reports to the Section in a timely manner. 8. The groundwater and surface water monitoring network for the closed MSW landfill is comprised of four groundwater monitoring wells (#MW-1, #MW-2, #MW-3, and #AMW-1), and two surface water monitoring locations (#SW-1 and #SW-2). During this inspection, all groundwater and surface water monitoring locations at the facility were observed. Continue to maintain access for all environmental monitoring locations at the facility. (Note — see Table #1 of this inspection report for additional information on groundwater monitoring well status at the facility.) The facility's LFG ventilation system currently includes five passive LFG vents, designated #GV-1 to #GV-5 (photo #1). The Section recommends the facility label the five LFG vents. The facility's LFG Monitoring Well system currently includes seven monitoring well locations (#GP-1B and #GP-1 to #GP-6) situated around the perimeter of the landfill. During this inspection, all LFG monitoring well locations were observed. Access to LFG monitoring locations was maintained. (Note — see Table #2 of this inspection report for additional information on LFG monitoring well status at the facility.) During this inspection, ground cover appeared to be well established on the slopes and cap of the closed MSW landfill (photos #2 & #3). As discussed post -inspection, continue to maintain ground cover on the areas where LFG vent installation activity was recently conducted at the facility, including the area around LFG vent #GV-5 (photo #4). After mowing events occur, continue to ensure the landfill cap and side slopes are inspected for subsidence, cracking, settlement, wetness, signs of erosion or damage, and that repairs are conducted as necessary. Although vent recently installed in November 2019, any bare areas should be re -seeded as necessary. 11. As discussed post -inspection, in accordance with the approved Post -Closure Care Plan - Section 3.4 (DIN 28399), remove any dead trees from the landfill cap as necessary (photos #5 & #6). As also discussed, continue post -closure inspections and maintenance of the closed landfill, per the approved Post -Closure Care Plan. 5. Continue to maintain all erosion control measures at the facility as necessary including rip -rap channels, sediment detention basins, permanent diversions, and inlet/outlet stabilization structures.
