OBSERVED VIOLATIONS: Rule .0835(b) Vehicle#FC26, tag#FB8959, VIN# correct per the firm application, 900 gallon capacity - was not available for inspection on the last visit. The firm name (Forever Clean) was damaged on both sides of this vehicle. Correct the firm name on both sides & submit a photo to this office by March 31st (include a ruler in the photo for reference & show both sides of the vehicle). Besides this discrepancy the lettering was correct on both sides of the vehicle. The tank appeared structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. SEE BACK OF PAGE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Mix up on vehicles during last visit, tag#FB8959 was not wrecked – inspected today. Tag#HW2071 must be the one that was wrecked & no longer in service. This completes inspections on all the vehicles listed on the firm application submitted for the 2020 permit. 2. Reviewed the truck logs at this time (example logs sheets currently being used). Discussed that the septic tank jobs shall show the complete physical address, the name of the particular WWTP being used for disposal shall be recorded and in particular a separate line shall be recorded for when waste is removed from the storage tanks to be taken for disposal at the WWTP (currently only recording when the waste is discharged into the storage tanks). 3. Investigated complaint regarding possible violations from runoff from hose washings at the rear of the site. Inspected area at the back of the site where the portable toilets are washed. Discussed cleaning procedures for the portable toilets & hoses and that any rinse water from the waste tanks or the hoses shall be contained (no discrepancies noted). No signs of any septage residuals on the ground surface in this area. No evidence of any septage violation in regards to this complaint.
