The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
On March 12, 2020, Chuck Kirchner conducted a comprehensive inspection of the Triangle YWN on University Drive
in Burlington, Alamance County
1. The yard waste notification (YWN) is located on a parcel of land at the corner of University Drive and
Huffman Mill Road (on the left from I-40, exit 140), across from the Alamance Regional Medical Center.
2. The primary use of the property is for Triangle Paving and Grading, Inc. equipment storage. The YWN is located on the right side of the lot.
3. The YWN site is used exclusively by Triangle Paving and Grading, Inc. and is not open to the public.
4. The area adjacent to the YWN contains stored aggregate materials.
5. Mr. Cagle stated that the material is to be ground and used as a soil amendment once enough material is received on site to grind. No ground material was observed.
6. Mr. Cagle previously stated that much of the LCD was received from a nearby construction project.
7. Ensure only woody debris material is accepted. Please contact this office prior to accepting any leaf or grass material as this would fall under composting regulations.
8. Mr. Cagle stated that the woody debris is to be ground and used as a soil amendment.
9. Ensure processed and unprocessed material on site is less than 6000 cubic yards quarterly.
10. Records should be maintained of the amount of LCD accepted. 11. Open burning of waste is prohibited. 12. Fires shall be reported to the Division orally within 24 hours of the incident and in writing within 15 days of the incident.
13. Ensure adequate fire lanes are maintained.
14. Access roads are of all-weather construction.
15. The facility is secured by a locked gate.
16. The facility has a proper sign.
