This report is prepared in accordance with North Carolina Department of Environment and Natural Resources (NCDENR) directives. 2.0 SITE ASSESSMENT 2.1 Site Description The subject property is located at 423 Pitts School Road Northwest in a mixed -use area within the jurisdictional limits of Concord, North Carolina (Figure 1). The site is improved with a three story building housing a Distribution Technology - Wal-Mart Consolidation Center # 7976 and an asphalt parking/access area. A site plan is included as Figure 2. A fiberglass 20,000-gallon underground storage tank (UST) containing diesel fuel was inadvertently punctured by a track -hoe bucket during tank tightness testing activities in March 1995. It was discovered that approximately 5,000 gallons of fuel were unaccounted for and were presumed released to the environment. Non -aqueous phase liquid hydrocarbons (free product) have been encountered in five groundwater monitoring wells since 1996. Monitoring well MW-02 and recovery well RW-02 have consistently contained free -phase petroleum. An aboveground storage tank (AST) used for storage of recovered free product is currently present at the site. The site is bordered to the east by Pitts School Road Northwest. The majority of parcels in the area are zoned residential and/or commercial. Municipally -supplied water provided by the City of Concord located at 850 Highway 601 South in Concord, NC is utilized by properties in the area. Since May 1996, a total of 844.96 gallons of free -phase petroleum have been removed from the site using a flexible axial peristaltic pump system (FAP), aggressive fluid vapor recovery events, pneumatic active skimmers (PAS), passive skimmers, mobile multi -phase extraction (MMPE) event, and hand bailing. Starting in January 2013, CES initiated surfactant enhanced free product recovery events at the subject site. To date, two injection / extraction events have been conducted resulting in approximately 680-gallons of surfactant enhanced water being injected and approximately 1,182- gallons of impacted groundwater being extracted and transported offsite for disposal. 3.0 SUMMARY OF ACTIVITIES CONDUCTED DURING 2018 3.1 February 2018 Free Product Recovery Event CES mobilized to the subject site on February 27, 2018 to evaluate free product thicknesses in monitoring / recovery wells MW-1, MW-2, MW-3, MW-4, MW-5R, MW-8, MW- 10, RW-1 and RW-2. MW-1 was unable to be gauged due to a tracker trailer being parked on the well. Free product was identified at monitoring / recovery wells MW-2 and RW-2 and approximately 28-ounces of free product was hand bailed. Groundwater elevation data is presented in Table 1. A summary of free product recovery for the February 2018 event is presented in Table 2. CES@NCa 2018 Annual Report Former Fredrickson Motor Express — Southern Release Area Concord, North Carolina Incident #13944 3.2 November 2018 Free Product Recovery / Groundwater Monitoring Event In accordance with NCDENR directives, CES performed a groundwater monitoring event to assess current dissolved -phase contaminant levels and to confirm the presence or absence of free -phase petroleum on November 12, 2018. CES mobilized to the subject site to collected groundwater samples from monitoring / recovery wells RW-1, MW-1, MW-2, MW-3, MW-4, MW-5R, MW-8, and MW-10. Free -phase petroleum was observed at monitoring well MW-2, and MW-1 and RW-1 were unable to be accessed, therefore, groundwater samples were not collected. Prior to purging and sampling, a Heron InstrumentsTM H.01L interface probe was used to record the static water level and to investigate for the presence of free -product at the monitoring / recovery wells. CES recorded the static water level using the interface probe from a reference mark scribed at the top of the well casing. Based on historic site assessment information, groundwater flow direction across the area of concern is to the southwest. CES utilized dedicated polyethylene bailers to hand bail approximately 50-ounces of free product from MW-2. Recovered product was transferred to the on -site holding tank pending future offsite disposal. Following removal of free product to the greatest extent possible, CES installed PIG® Monitoring Well Skimming Sock in MW-2. The skimming sock is capable of absorbing up to 17-ounces of free product and will be replaced semi-annually. Free product recovery data for the November 2018 event is presented on Table 3. Prior to sampling, CES purged the wells to remove suspended solid material from the water column and to collect samples representative of aquifer conditions. The wells were purged until a minimum of three well volumes had been displaced. The groundwater samples were collected using dedicated, disposable, high -density polyethylene (HDPE) bailers and were transferred directly into the appropriate sample containers immediately upon collection. Disposable, nitrile gloves were used during all phases of sample collection. The groundwater samples were labeled and maintained on ice pending delivery to Pace Analytical (Pace) a NC certified laboratory for volatile organic compounds (VOCs), MTBE, and isopropyl ether (IPE) via Standard Method (SM) 6200B. Strict sample security and chain -of -custody documentation were maintained during all phases of transport. A summary of groundwater quality data is included as Table 4. Chain -of -custody documentation and the laboratory Certificates of Analyses are included as Appendix A. 4.0 SUMMARY Laboratory analyses performed on the groundwater samples collected during this investigation yielded exceedances of 2L Groundwater Standards in the sample collected from recovery well RW-02. Free product was identified during the referenced sampling event at monitoring well MW-2. A free product absorbent boom was replaced at MW-2 and will serve as an interim method of free product removal. Based on the revised reporting schedule, as approved by NCDENR on September 30, 2016, the next event will consist of free product gauging and bailing and is scheduled for April 2018.
