with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. This site previously was SLAS #04-08 under L & L Environmental Services LLC. Last year L & L
Environmental was sold (the business & the septage treatment facility in Charlotte) and this site in Anson
County was permitted under SLAS # 04-09 for Liquid Waste Inc. No septage has been brought to this site
since the business sale of L & L Environmental. No land application has occurred since last year.
2. This current permit only covers Field #2 (6.5 acres) as per Mr. Oaks request. Land application may occur on
this field if there is an adequate fescue cropstand & field markers are adequately posted. Currently there are
missing field markers, but the cropstand looks adequate at this time.
3. Discussed that possibly in the future other fields that were previously permitted may be added back into the
SLAS permit. This will require soil samples and an established fescue cropstand on these other areas. A SLAS
application & resigned nutrient management plan will be required for the new permit.to include any additional
fields. A site visit to measure setbacks/post markers & GPS the areas will also be required.
4. The required signs are posted and the access road was in fair condition (recent rains).
5. Did not drive over entire site due to recent rains. Noticed disturbed areas that may be animal activity (hogs?).
6. Discussed that the required 3 hours of land application training will be obtained each year for the current SLAS
permit. Mr. Oaks stated that a representative would be attending the Morganton class in March. Currently there
is no firm permit (for septage pump trucks) that would require the 4 hours of firm training yearly. Discussed the
current non pumper status for this site/ fee & form to be submitted yearly to the Raleigh Central office.
7. A soil sample shall be taken by the operator this fall on the current permitted field as per Rule .0843(b).
