The lettering shall include the firm name, town name, phone number and permit number as shown on the firm application. All lettering shall be no less than three inches in height and legible. Inspected vehicle tag#HT2535, VIN# correct per the firm application. The town name (Fuquay Varina) was missing. Inspected vehicle tag#EY9563, VIN# correct per the firm application. The firm name (Forever Clean) was damaged. Correct the lettering on these two vehicles – a follow up inspection will be performed. The tanks appeared structurally sound on both vehicles and securely attached. Outlet covers were on and no drips/leaks were noted. SEE BACK OF PAGE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Tweaked the recent firm application to reflect the correct firm name (Forever Clean) that is in compliance (in regards to lettering height) on the trucks. Inspected 11 vehicles at this time. 2. Tag#CH2856, VIN# correct per the firm application. This vehicle is not in use & may be sold. Tag#HY2890, VIN# correct per the firm application. Tag#FF2518, VIN# was incomplete on the firm application – should be “1FDUF5GT2GEA73604” Tag#JE5867, VIN# correct per the firm application. Slight drip noted at outlet but was corrected during visit. Tag#FB8996, VIN# correct per the firm application. Tag#JP1368, VIN# incorrect on firm application, was “3C7WRMDK5KG546659” and 1100 gallon capacity. Tag#HV7451, VIN# correct per the firm application. Tag#HL7912, not listed on the firm application. VIN#5PVNJ8JLXH4551929 and 1600 gallon capacity. Tag#HN6474, VIN# correct per the firm application. These nine vehicles were in compliance in regards to lettering, outlet covers on, no drips/leaks and the tanks appear structurally sound & securely attached. 3. One truck listed on the firm application (Tag#HW2071) was in the shop at this time and not available for inspection. Another truck listed on the firm application (Tag#FB8959) was wrecked & will not be repaired. 4. The follow up inspection will include inspections on the two vehicles that needed lettering corrections, the vehicle that was in the shop during this visit and also a review of the truck pumping logs. 5.
