OBSERVED VIOLATIONS:
1. Inspected vehicle tag#JX5125, VIN# correct per the submitted application. The correct permit number (00384) &
town name (Stedman) are not displayed on both sides of the vehicle in accordance with Rule.0835(b). Correct the
lettering as needed & schedule another inspection.

The tank on this vehicle appeared structurally sound & securely attached. Outlet covers were on and no
drips/leaks were noted.

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1- Inspected vehicle in regards to Mr. Carter obtaining a firm permit again (there was no firm permit for a couple
of years). Mr. Carter attended the new firm operator class in January 2020 and has submitted an application &
fee to the Raleigh Central office. Mr. Carter is currently working with PWC to obtain a disposal authorization.
2- No septage pump jobs may be performed until the firm permit issued by the Raleigh Central office.
