1) Bermuda grass in the Field has fallen below 80% coverage during inspection. This is a violation of Rule .0838(a)(18) that states “Approved nutrient management plans shall be followed.”

General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1 - Site has proper signage 2 – Good access road 3 – Markers were not seen on field 4 – No signs of septage discharge on field
5 – In the above pictures the Bermuda grass coverage of the Field has fallen below the stated 80% of the permitted Nutrient Management Plan. This is a violation of Rule .0838(a)(18). This is a reoccurring issue from
SLAS1021_NOV_20180802 and SLAS1021_NOCV_20190710. Permit in process of closure.
