Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Inspected vehicle tag# AS3887, VIN # correct per the current firm application, 1,400 gallons. The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of this vehicle per the information on the current firm application. However some of the lettering on the passenger side was damaged – still legible at this time. 2. Inspected vehicle tag# JL9295, the VIN# on the firm application was incorrect – it is J8BE5B145Y7900116, this needs corrected on future applications. 650 gallons. The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of this vehicle per the information on the current firm application. 3. Reviewed the records for the porta john routes – it appears that not all the required information is being maintained at this time. Discussed that the truck logs for the porta john routes shall contain the following info. - The date of the route (this is the servicing of the porta johns, not the placement) - The approximate gallons of waste (for that route by that truck on that day) - A name for that route (for the location of the septage) - The date that waste comes off that truck (could be that day or a couple of days later) - The disposal location of the waste (Mr. Hurley stated that all porta john waste goes to Mt. Gilead). I gave Mr. Hurley an example truck log for his use. Please revise the log for the porta john routes to record the required information. The truck logs for the septic tank jobs were reviewed on the previous inspection of 1/31/2020 and all the required information appeared to be present for those pump jobs. 4. Thank you for your time.
