Pleasant Road Concord, North Carolina 28025 Waters Edge Job No. R19-21 Dear Mr. Hall: Waters Edge Environmental, LLC (Waters Edge) is pleased to present the attached Initial Assessment Report (IAR) for the August 8, 2018 diesel fuel release for DLB Trucking, Inc. near the residence located at 6777 Mt. Pleasant Road in Concord, North Carolina (see Figure 1). Based on the results of the investigation, one of the two TPH-DRO results were above the NCDEQ threshold of 100 mg/kg. As such, we would conclude that additional time will be required to allow for the bioremediation process to lower the TPH-DRO levels at the spill area. The release has been reported to NCDEQ. If you require additional information concerning the attached IAR, please contact us at 919.859.9987. Sincerely, WATERS EDGE ENVIRONMENTAL, LLC Phillip L. Rahn P.G. Diesel Release 6777 Mt. Pleasant Road Concord, North Carolina 28025 May 8, 2019 1 Introduction Waters Edge has prepared this Initial Assessment Report (IAR) for the diesel fuel release which occurred near a residence located off 6777 Mt. Pleasant Road in Concord, North Carolina (see Figure 1). This IAM Report follows the NCDEQ UST Section Guidelines for Investigation and Remediation of Contamination from Non -UST Petroleum Releases -with the site information cover sheet contained in Appendix A. We have also included UST-62 24-hour Notification of Discharge Release Form also in Appendix A. Herein, are the remaining sections of the report. 2 Initial Assessment Report 2.1 Site History and Characterization 1. List, describe, and indicated location of AST systems and of all spill or other non -UST releases- This release occurred due to a February 6, 2019 DLB Trucking, Inc. truck accident which resulted in the discharge of an estimated 120 gallons of diesel fuel near 6777 Mt. Pleasant Road in Concord, North Carolina (see Figure 1). This release discharged onto the ground near the road (see location in Figure 2). The regional area would be considered residential. Describe initial abatement activities, assessment activities and corrective action performed to date and list all reports previously submitted- There were no previous reports. On February 6, 2019, Shamrock Environmental mobilized to the site and applied bioremediation amendment to the spill area which measured approximately 30' (L) x 10' (W) x 1.0' (D). On April 10, 2019, Water Edge mobilized to the site. We collected two (2) samples with a hand auger (SS-1 and SS-2-see Figure 2). The samples were collected at 0.5' bgs with a hand auger and submitted to the laboratory according to EPA -accepted procedures. Based on DEQ guidance, this sample was analyzed for diesel -ranged total petroleum hydrocarbons (TPH-DRO) according to SW-846 Method 8015 (results summarized in Table 1 and documented in Appendix B). Page 1 of 2 Initial Assessment Report DLB Trucking, Inc. Diesel Fuel Release 6777 Mt. Pleasant Road Concord, North Carolina 28025 May 8, 2019 3 Conclusions and Recommendations Based upon the soil assessment findings, we would conclude the following: • While one sample was ND (SS-1), SS-2 detected 276 mg/kg TPH-DRO which is in excess of the NCDEQ threshold of 100 mg/kg. As such, we would conclude that additional time will be require for the bioremediation to reduce the impact levels below the TPH-DRO DEQ threshold. The release has been reported to NCDEQ.
