Active, Life of Site Permit, Issued February 20, 2017. Combined with 2803-CDLF-1995. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS:
None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 – Facility has required signage. Locking gates and fencing for security. 02 – Interior roads are constructed of all-weather materials and well maintained.
03 – This facility has 8 Transfer Station Operations Specialist available: (Bowen) Kelvin Wrighton, expires 11/5/2021, Mike Chesson, expires 5/18/2021, Martyn Mitchell, expires 11/14/2022 (Dare County) Shanna Fullmer, expires 10/9/2020, Doug Huff, expires 5/18/2021, Deborah A. Equils, expires 11/3/2022, Thomas Tillett, expires 10/9/2020 and Jonathon DeWeese, expires 11/15/2021. 04 – Screening records were reviewed. Records were up to date and had recorded items pulled out of waste stream. 05 – Reviewed several incoming load tickets and they had required information. 06 – Facility has fire extinguishers and 1inch water hoses for fire control. All in good working order. 07 – This inspection was conducted during working hours. 08 – Overall tipping floor looks good. Clean behind push walls. 09 – The waste deflectors are scheduled to be replaced due to the amount of waste falling onto the load out bay floors. 10 – Floor drains in load out bays were cleaned out and working well during this inspection. 11 – Windblown trash on the inside of the chain link fence and around the facility was at a minimal. Most of this windblown trash is outside of the chain link fence. Needs to be removed DAILY. 12 – The leachate holding tank is pumped out weekly. Please keep a copy of these records available onsite.
13 – The office where the records are kept was the clean. 14 – Permit issued 2/20/2017 was available and reviewed. Operations Plan also reviewed. 15 – Trailer loading area looks good today. One trailer was leaking during this inspection. Just pulled out and a
trough was placed under it to capture the leachate. In need of repair. 16 – There was windblown trash inside the containment area of the leachate tank that needs to be removed and checked daily. 17 – All items on this inspection report were discussed with participants listed on this report. 18 – Overall, this facility is looking to be managed better. 19 – Digital Pictures were taken during this inspection. Some are attached to this report.
