Incident Number: 19503
Risk Classification: High
The UST Section, Division of Waste Management, Mooresville Regional Office has not
received a monitoring report for the above mentioned site since the June 26, 2017 Groundwater
Monitoring Report. A groundwater monitoring event shall occur as soon as possible with the
Groundwater Monitoring Report submittal due to this office by May 31, 2020.
Please note that sampling and correction action must be conducted in accordance with the
Guidelines for Assessment and Corrective Action for UST Releases. Failure to comply in the
manner and time specified may result in the assessment of civil penalties and/or the use of the other
enforcement mechanisms.
If you have any questions regarding trust fund eligibility or reimbursement from the
Commercial or Noncommercial Leaking Petroleum Underground Storage Tank Cleanup Funds,
please contact the UST Section Trust Fund Branch at (919) 707-8171. If you have any questions

I've been looking through the UST file for Lake Fisher Pump House (Incident #19503). The last groundwater
monitoring report that we have in our file is dated June 21, 2017. Brett Morris sent a Notice of Regulatory
requirements on June 26, 2017 to continue annual monitoring at the site. Do you have annual groundwater
