The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 - This facility was issued a Life of Site permit on August 4, 2015 and expires on October 11, 2025. 02 - Facility has adequate signage.
03 - Facility is secured by chain link fences and locking gates.
04 - Interior roads are made with all-weather materials. 05 - This facility has 3 Certified Transfer Station Operations Specialist: Tyrone Sumler, expires 12/7/2020, Ricky Knight, expires 6/20/2021, Melvin Brown, expires 8/15/2021 and Andrew Ward, expires 6/21/2022. All 3 certificates
were in the screening record book. 06 - Screening records were reviewed and were found up to date. 07 - The Leachate drains at the Transfer Station looked good today.
08 - The Leachate tank is in need of pumping out to ensure adequate space for leachate. 09 - This facility has employed a new contractor. Was CEI. Now Stella Environmental Services. 10 - One CEI transport trailers (# T44876) has been left on-site and is loaded. Rear seal is leaking. Mud flap had caused a small amount of leachate to bypass the containment trough. Needs to be removed and leachate cleaned up. John College was notified that the trailer has been removed. SEE ATTACHED PICTURE. 11 - Repairs on the Outside metal have been done to keep trash inside the building. 12 - The windblown trash is getting better. Small amount during operating hours still behind the chain link fence. Staff was removing this at the end of the inspection. 13 - PTO was issued 8/4/2015. A 5 year limited review will be due by 10/11/2020. PTO expires 10/11/2025. 14 - Waste from this facility is permitted to be taken to Sampson Co. Disposal 8202-MSWLF-2000, East Carolina Regional Landfill, 0803-MSWLF-1993 and Brunswick Waste Management Facility, SWP 583 in Lawrenceville Va. 15 - Facility is permitted to receive waste from within, Duplin, Edgecombe, Franklin, Greene, Halifax, Johnston, Lenoir, Martin, Nash, North Hampton, Pitt, Wake, Wayne and Wilson Counties. 16 - Facility is designed to receive 1000 tons per day. 17 - Scale house tickets have required information. 18 - Digital pictures were taken during this inspection and some are attached to this report.
