Dear Ms. Beverly,
As described to Shield Engineering, Inc. (Shield), on December 20, 2018 there was an overfill of an unknown
amount of approximately 35 to 50 gallons of heating oil #2 from a fuel delivery truck to an underground
storage tank (UST) at the Rocky River Presbyterian Church at 7490 Rocky River Road in Harrisburg, North
Carolina. The overfill caused heating oil #2 to come out of the vent pipe of the UST and displace onto the
ground. Unfortunately there was a rainfall event at the time which caused the heating oil to spread away from
the vent pipe area. Notification was made to the local fire department and subsequently to the North Carolina
Department of Environmental Quality (NCDEQ). (See the release report in Appendix A). The local Fire
Marshall also visited the site. Carolina Oil Company mobilized a cleanup crew to the church property on
December 24, 2018 to start excavation activities. Shield was requested to document the cleanup on the next
two cleanup days December 26 and 27, 2018.
Personnel from Shield's Risk Management Group arrived at the site on December 26, 2018 and assisted in
determining the extent of impacted soils remaining. We determined that there was evidence of an historical
release or releases of heating oil into the soil. We also determined that due to the UST still being in place, two
adjacent air conditioning units, and underground utilities that there were limits to the vertical extent of cleanup.
The release may have migrated slightly into the cemetery and the excavation in that direction was rightly
stopped by church personnel. The release had also entered into the furnace room through the brick wall
located adjacent to the UST. Within the furnace room, there are two floor drains that the release may have
entered. The area of discharge for these drains is unknown. On December 24 and 26, 2018 contaminated soil
was excavated and transported to Carolina Oil Company's property for staging prior to proper disposal.
Shield collected post -excavation confirmation soil samples during cleanup activities. On December 27, 2018
the oil stained walls, floor, and floor drains within the furnace room were sprayed with bioremediation fluid.
On December 27, 2018 the source area was also sprayed with a bioremediation fluid, covered with plastic
sheeting to restrict rainwater from migrating contamination from the source area impacted soils, and the entire
excavated area was backfilled. The results of the samples' analyses are "clean" below the North Carolina
Action Levels.
We recommend that when the UST is removed, which is planned by the church, that the historic release(s) and
the vertical extent of soil contamination get cleaned up.

Soil Excavation
Figure 1 shows the church's location. The latitude and longitude coordinates of the spill area are noted in the
subject line above. On December 24 and 26, 2018, soil was excavated by the cleanup crew dispatched by
Carolina Oil Company. Discolored grass and an organic vapor analyzer (OVA) were used to determine the
extent of contamination. The OVA was received from, and calibrated by, Eastern Solutions, LLC. The
cleanup crew used a mini -excavator and shovels for excavation activities. Shield assisted and documented the
cleanup. The excavation was screened with the OVA to assist in documentation and communication to the
cleanup crew when the petroleum impacted soil had been removed to a reasonable extent.
During the excavation work, grey -green, old rancid smelling soil was found in the vicinity of the fill and vent
pipes of the UST indicating that an historic release or releases had occurred. Where this historic release(s)
impacted soil comingled with the new release it was excavated. In areas around the air conditioning units and
close to the church wall where only the historic release(s) was/were evident, soil was not excavated. Also
when the top of the UST was encountered or the two underground electric utility lines in the area were
encountered, additional soil was not excavated. The release also may have migrated slightly into the cemetery
area and the excavation in that direction was rightly stopped as directed by church personnel. The soils within
the source area impacted by the new release which could not be excavated were sprayed as requested by church
personnel on December 27, 2018 with BioRem 2000, a microbe -powered bioremediation cleaning agent.
The final dimensions of the irregularly shaped excavation were approximately 90 feet long (east -west) by
approximately 20 feet wide and up to 3 feet in depth. Four post -excavation soil samples (S-1 through S-4)
were taken from areas representative of the cleaned up area. The sample locations can be seen on Figure 2.

Post Excavation Samnline and Analyses
The post excavation confirmation samples were collected from the base of the excavation, each in a 0-6 inch
interval. The samples were placed in laboratory containers and transported on ice under Chain -of -Custody
protocol to Pace Analytical Services, Inc. (Pace) a North Carolina Certified laboratory for analyses.
The samples were analyzed by Pace for total petroleum hydrocarbons (TPH) Diesel Range Organics (DRO)
and Gasoline Range Organics (GRO) using EPA Method 8015. These are the heating oil #2 parameters as
determined by the NCDEQ. Extra sample volume was collected for potential risk -based analyses, to be
analyzed as necessary based on initial TPH results.
Laboratory results are included in Table 1 and Appendix C. All of the results were non -detect except one
sample (S-2) which showed 33.8 milligrams per kilogram (mg/kg) of TPH DRO. The NCDEQ Action Level
for TPH DRO is 100 mg/kg, so the S-2 sample is well below the NCDEQ Action Level. Since none of the
samples were above the NCDEQ Action Levels, the samples were not additionally analyzed for the risk based
analyses.
Disposal and Site Restoration
The excavated soil was taken to Carolina Oil Company for staging and is to be properly disposed. After
excavation and spraying BioRem 2000 on the source area, the source area was covered with plastic sheeting as
requested by church personnel, to restrict rainwater from migrating contamination from the source area
impacted soils, and then the entire excavated area was backfilled. The site work was completed on December
27, 2018. The area was to be seeded and covered with straw for erosion control.
Limitations
Shield performed these environmental services at the subject site on behalf of Carolina Oil Company. Shield
has performed this scope of work as an independent contractor/consultant using reasonable care and skill in
accordance and consistent with customary industry standards of engineering, geology, and hydrogeology

practices. This standard of care is the sole and exclusive standard of care that can be applied to measure
Shield's performance of the work. No other warranty, expressed or implied is made or intended by Shield.
All recommendations, findings, and conclusions, made by Shield have been made to the best of Shield's
knowledge, opinion and belief, based upon information obtained during this scope of work, and is limited by
the scope nature and type of services as agreed upon between Carolina Oil Company and Shield. Conclusions
are provided with the understanding that Shield is presenting information and not rendering legal advice. If
such advice is needed, legal counsel should be consulted. It is the responsibility of Carolina Oil Company
under advice of its counsel to notify the appropriate local, state, and/or federal public agencies as required by
law; or otherwise to disclose in a timely manner, any information that may be necessary to prevent damage to
human health, safety, or the environment.
Compliance with recommendations provided as part of this report in no way assures compliance with local,
state and/or federal laws, regulations, and/or requirements. Information was obtained from Carolina Oil
Company, Rocky River Presbyterian Church personnel, and Pace Analytical Services, LLC. This information,
to the extent that it was relied on to generate this report, is assumed to be correct and complete. The work
performed in conjunction with this report and the data developed are intended as a description of available
information at the dates and specific locations given. Shield is not responsible for inspecting, examining, or
reporting findings or recommendations with respect to any conditions that were knowingly or unknowingly
withheld, concealed, hidden, or in any way not disclosed or observable at the time of this scope of work.
Conclusions
Initial stabilization and abatement of the spill occurred on December 24, 2018. Continued excavation of
contaminated soil occurred on December 26, 2018. The post excavation analytical results were "clean," either
non -detect or below the NCDEQ Action Level using TPH-DRO and GRO analyses. The soils within the
source area impacted by the new release which could not be excavated due to encountered restrictions were
sprayed with a bioremediation fluid, as was the oil in the furnace room. The source area was then covered with
plastic sheeting to restrict rainwater from migrating contamination from the source area soils, and the entire
excavated area was backfilled.
The excavated soil was removed for proper disposal and the area has been restored. Shield recommends that
when the UST is removed, which is planned by the church, that the historic release(s) and the vertical extent of
soil contamination get cleaned up. If you have any questions regarding the content of this report, please

I just wanted to touch base with you on this site since both parties, Carolina Oil and Rocky River Presbyterian
Church, are trying to keep open correspondence between each other on any further requirements for this
project. The hardcopy of our report should go out today so I'm assuming it will get to your office early next
week. Based on the knowledge that you have on this release and other information that has been conveyed to
you by David Stoner, do you currently have an idea of steps that may be required, if any, to move this project
toward closure or will you wait until you review the hardcopy to make any decisions? If the latter of the two,
what would be the rough timeframe on receiving your decision for course of action once you have reviewed the
report?
