No permit required PURPOSE OF SITE VISIT: Follow up facility compliance inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - While in the area, John College decided to inspect this Notified LCID facility. 02 - Facility has locking gate for security. 03 - Signage has required information. 04 - DS08-010 is also located on this parcel. 05 - Access road is constructed of all-weather materials. 06 - Matt Wilson was called to notify him of this un-announced inspection. A voicemail was left for Matt. Matt returned the call after John College had left the locked facility. 07 - A scheduled facility inspection will be conducted in the near future to follow up on the progress of the following items listed on the last facility inspection report conducted on August 08, 2019; (a) Side slope work is still in process to meet 3:1 side slopes. Need to take advantage of dry weather to accomplish this task. This project is outsourced to a contractor. (b) Need to find a source of cover material to ensure ALL WASTE IS COVERED AT LEAST MONTHLY. Has minimal stockpiled cover material on the site. (c) There was a small amount of non-conforming waste (PLASTIC) mixed in the LCID waste. 08 - This facility is still heading in the right direction to achieve compliance.
