See Figure 1, Structural Fill Location Map, for location of ash fill and previous area of ash fill that is now dirt fill (see additional comments for more information). None Page 1 of 9 Revised 2019 supplemental SF site inspection STRUCTUAL FILL COAL ASH INSPECTION Additional Comments: History: On June 10, 1996, Reuse Technology, Inc, submitted written notice to DEQ of a coal combustion by- product structural fill project at the 2700t" block of N. Wesleyan Boulevard, Rocky Mount, NC 27803 (Northside Business Park. This project was estimated to use approximately 41,000 tons of coal combustion by-product from Cogentrix facilitates in Rocky Mount. On July 10, 1996, DEQ sent Reuse Technology, Inc. a letter stating that the Notification requirements concerning beneficial use of coal combustion by-products had been satisfied. On July 8, 1998 Reuse Technology, Inc. sent a letter and map to DEQthat stated, "To date, CCBs have been placed in two areas within the park. At the southern end of the park CCBs were placed as structural fill for a building pad and parking lots. Several structures have been constructed on this fill material. At the north end of the park a berm was constructed with a CCB core. The berm was constructed to provide a visual and sound barrier for the adjacent residential neighborhood. No further CCB placement is planned for this park at this time." On July 14, 1998 DEQ sent Reuse Technology a letter and map (Figure 1) notifying them that the project is in non- compliance with Section .1700 of the NC Solid Waste Management Rules. On November 30, 1998 Reuse Technology sent DEQ a letter stating, "Corrective actions for the coal combustion by-product fill in the berm at the north end of Northside Business Park have been complete. All coal combustion by-products were removed from the berm." On December 11, 1998 Reuse Technology submitted a notice of completion for the Northside Business Park coal combustion by-product structural fill project.
