STATUS OF PERMIT:
Current permit issued June 2, 2016; expires November 23, 2020. The request for a permit amendment (renewal) is due
no later than May 23, 2020.

ADDITIONAL COMMENTS
1. The hours of operation are Monday thru Friday, 7:30am-12:00pm and 12:30pm-4:00pm; closed weekends.
2. The facility has proper signage.
3. The facility is properly secured with a locked gate. Personnel are onsite during operating hours.
4. The permit and operations plan were reviewed. All documentation was available and presented in an orderly
manner.
5. This facility is permitted to receive solid waste generated within Columbus, Brunswick, New Hanover, Pender,
Bladen, and Robeson Counties. Waste transported for disposal must go to Sampson County Disposal (Permit
Number 8202-MSWLF-2000) in Roseboro.
6. Staff training records were reviewed:

7. Tire certification records well organized and current through 12/2/2019.
8. Hilco Transport loads and hauls the waste for this facility.
9. Tires are being placed in a covered trailer upon receipt.
10. Windblown debris is removed daily as required.
11. The siding of the structure has been damaged in multiple places along the floor and at the top of the push wall. This is
allowing waste to exit the structure and tipping floor. This was discussed with Hilco and County staff as well as
documented in previous inspection reports. Multiple bids have been obtained and contracts are being reviewed. Be
sure to remove all waste that exits at these openings.
12. The leachate collection system was reviewed and is operating properly at this time.
13. The contingency plan has been updated with current phone numbers and posted in offices and in the scale house
14. There were several charged fire extinguishers available for fire control.
15. The access around the facility is stable and no soil erosion was noted.
16. Digital pictures were taken during the inspection.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
