The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected four vehicles – three were lettered under the name of Brian’s Waste Recycling, one only had the
name of Enviro Choice. Mr. Hunt indicated that with the 2020 firm permit Brian’s Waste Recycling may
be dropped/removed from the vehicles. If this occurs then another inspection will be needed to inspect the
Enviro Choice name on the vehicles. Inspected the following vehicles.
Vehicle tag#YA131930, VIN# correct per the 2019 firm application. Lettering (slightly damaged decals) correct
on both sides of the vehicle (Brian’s Waste Recycling). Outlet covers were on & no drips noted. The tank
appeared structurally sound & securely attached.
Vehicle tag#YA131928, not listed on the 2019 firm application because this vehicle is not in use. Lettering
correct on both sides of the vehicle (Brian’s Waste Recycling). The tank appeared structurally sound & securely
attached. Outlet covers were on and no drips noted.
Vehicle tag#AE-57684, correct VIN#1L98T41241G22309 (slight mistake on firm application). Lettering is
correct on both sides of the vehicle (Brian’s Waste Recycling). The tank appeared structurally sound & securely
attached. Outlet covers were on and no drips noted. This is a new tanker.
Vehicle tag#YA131927, correct VIN# correct per the 2019 firm application. This vehicle only had “Enviro
Choice” displayed on the lettering – otherwise the lettering was correct on both sides of this vehicle. If
Brian’s Waste Recycling is to stay on the trucks in 2020 then the lettering needs to be corrected on this
vehicle. The tank appeared structurally sound & securely attached. Outlet covers were on and no drips noted.
2. A new vehicle may arrive in late Jan or February which will require an inspection. Please contact this office
at that time.
3. Vehicle tag#YA131929 as listed on the 2019 firm application was in the shop at this time & not available for
inspection.
4. Reviewed truck pumping logs – the required information is being maintained except it is not being
recorded correctly when waste goes directly from a pump truck to the field or to a WWTP – tweak the
logs to reflect when this does occur.
