Active, Life of Site Permit. Issued June 10, 2019 and expired May 17, 2055. This facility permit is listed as Perquimans,,Chowan,,Gates Landfill Commission (owner) Republic Services of North Carolina, LLC (operator).
PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection.

None

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 - This facility has a locking Gate at roadside entrance for security.
02 - Signage has required information. 03 - Facility is permitted to receive waste from Perquimans, Chowan and Gates Counties. 04 - Waste must be transported to The East Carolina Regional Landfill (0803-MSWLF-1993) in Bertie County. 05 - Windblown trash was not an issue. 06 - Loaded trailers had troughs under the rear door in the event of leakage. None noted. 07 - The tipping floor and loading bay were clean. 08 - Screening records were available and were up to date. 09 - Electronics are being collected at this site. Managed well.
10 - Scrap metal area is a bit scattered due to loader breakdown.
11 - Yard waste is being managed well. Has fire lanes, and temperature records are being recorded. 12 - Facility has 2 Certified Transfer Station Operations Specialist: Charlie Rome: expires 3/12/2022, David Lowe: expires 5/25/2022. 13 - Interior walls of Transfer Station have been washed since the last inspection. 14 - Scrap tires are currently on the ground due to the loader breakdown. Facility has adequate trailers available to hold the tires as soon as the loader is repaired. 15 - CFC removal records available and reviewed. 16 - PTO and approved plans onsite and available and reviewed. 17 - Load in and out records available and reviewed. Has required information. 18 - Land Clearing waste was being ground during this inspection. 19 - Concrete area looks good.
20 - Overall management of this facility is good. 21 - Digital pictures taken during this inspection.
