Dispenser: includes the dispenser and the equipment used to connect the dispenser to the piping (e.g., a release from a suction pump or from components located above the shear valve) Submersible Turbine Pump (STP) Area includes the submersible turbine pump head (typically located in the tank sump), the line leak detector, and the piping that connects the submersible turbine pump to the tank Delivery Problem: identifies releases that occurred during product delivery to the tank. (Typical causes associated with this source are spills and overfills.] Other: serves as the option to use when the release source is known but does not tit into one of the preceding categories (e.g., for releases from vent lines, vapor recovery lines, and fill lines) Unknown: identifies releases for which the source has not been determined Definitions of Causes Spill: use this cause when a spill occurs (e.g.. when the delivery hose is disconnected from the tank fill pipe or when the nozzle is removed from the dispenser) Overfill: use when an overfill occurs (e.g., overfills may occur from the fill pipe at the tank or when the nozzle fails to shut off at the dispenser) Physical or Mechanical Damage: use for all types of physical or mechanical damage, except corrosion (e.g., puncture of tank or piping, loose fittings, broken components, and components that have changed dimension) Corrosion: use when a metal tank, piping, or other component has a release due to corrosion (e.g., for steel, corrosion takes the form of rust) Installation Problem: use when the problem is determined to have occurred specifically because the UST system was not installed properly Other: use this option when the cause is known but does not fit into one of the preceding categories (e.g., putting regulated substances into monitoring wells) Unknown: use when the cause has not been determined Tim Collins Property, 2640 Shenandoah Ave, Charlotte, NC Date Printed: 11/27/2019 2:43:34 1 60 2 9 2 62 2608 r. 262 2 2 2 0 0 ,2 6151" 261 E 2641 02645 74 264-90 26530 0 2657 ■ 2660 • 02664 26680 0 2672 • 2137 7 fkr / 11 (11 e 4F 2 02721 'W/T , 2 .641 7W 27.29 6 0.274OtP 2624 4 274102737 41 2653 &749• 2 745 01 2657 40 N 26 'r 0, W E 0 0.005)01 0 02 Miles . 0 + S This map or report is prepared for the inventory of real property within Mecklenburg County and is compiled from recorded deeds, plats, tax maps, surveys, planimetric maps, and other public records and data. ND - Not Detected at or above adjusted reporting limit. TNTC - Too Numerous To Count J - Estimated concentration above the adjusted method detection limit and below the adjusted reporting limit. MDL -Adjusted Method Detection Limit. PQL - Practical Quantitation Limit. RL - Reporting Limit - The lowest concentration value that meets project requirements for quantitative data with known precision and bias for a specific analyte in a specific matrix. S - Surrogate 1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is a combined concentration. Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery and RPD values. LCS(D) - Laboratory Control Sample (Duplicate) MS(D) - Matrix Spike (Duplicate) DUP - Sample Duplicate RPD - Relative Percent Difference NC - Not Calculable. SG - Silica Gel - Clean -Up U - Indicates the compound was analyzed for, but not detected. Acid preservation may not be appropriate for 2 Chloroethylvinyl ether. A separate vial preserved to a pH of 4-5 is recommended in SW846 Chapter 4 for the analysis of Acrolein and Acrylonitrile by EPA Method 8260. N-Nitrosodiphenylamine decomposes and cannot be separated from Diphenylamine using Method 8270. The result reported for each analyte is a combined concentration. Pace Analytical is TNI accredited. Contact your Pace PM for the current list of accredited analytes. TNI -The NELAC Institute. LABORATORIES PASI-C Pace Analytical Services - Charlotte ANALYTE QUALIFIERS D3 Sample was diluted due to the presence of high levels of non -target analytes or other matrix interference. E Analyte concentration exceeded the calibration range. The reported result is estimated. IK The recalculated concentration of the calibration standard(s) did not meet method acceptance criteria; this result should be considered an estimated value. M1 Matrix spike recovery exceeded QC limits. Batch accepted based on laboratory control sample (LCS) recovery. N2 The lab does not hold NELAC/TNI accreditation for this parameter but other accreditations/certifications may apply. A complete list of accreditations/certifications is available upon request. SO Surrogate recovery outside laboratory control limits. S2 Surrogate recovery outside laboratory control limits due to matrix interferences (confirmed by similar results from sample re -analysis). S4 Surrogate recovery not evaluated against control limits due to sample dilution. S5 Surrogate recovery outside control limits due to matrix interferences (not confirmed by re -analysis). v1 The continuing calibration verification was above the method acceptance limit. Any detection for the analyte in the associated samples may have a high bias. v2 The continuing calibration verification was below the method acceptance limit. The analyte was not detected in the associated samples and the sensitivity of the instrument was verified with a reporting limit check standard. Maps of the site location and soil sample locations are included along with the results of the initial closure samples, collected on 11/8/2019. We are awaiting the results of over -excavation samples collected on 11/ 26/2019, and will follow-up with a letter report of the UST removal activities, and Notice of Residual Petroleum if needed for a Notice of No Further Action. From: Al Quarles Sent: Friday, November 22, 2019 9:14 AM To:'trudy.beverly@ncdenr.gov' <trudy.beverly@ncdenr.gov> Subject: Residential Heating Oil Tank Release Trudy, I have a residential site in Charlotte with a unregulated, non-commercial heating oil tank release. ATC is in the process of preparing a proposal for next actions, and I wanted to discuss this site with you to see if we need to add or subtract from our proposed activities. No water supply wells within 1,000 feet based on Mecklenburg County's Well Information System, and two site assessment for other USTs sites within 200 and 500 feet from this site. • A surface water body is located within 500 feet of the site. • Depth to groundwater likely 12-18 feet based on the two site assessments referenced above. Soil sampling was requested by the lender for a potential buyer of the property, and the property is in Bankruptcy court with the Trustees handling the UST Closure and assessment. The Bankruptcy court would like to move as soon as practical to obtain an NFA to sell the property, with a Notice of No Further Action. We just received the final results yesterday. One of two initial soil samples contained TPH-DRO at 5,010 mg/kg. Risk -based rule analysis indicated several exceedances of the Soil -to -Groundwater MSCCs, and three compounds (1-methylnaphthalene, C9-C18 Aliphatics, and C9-C22 Aromatics) exceeding the Residential MSCCs by 1.6 to 3.6 times. The second soil sample contained TPH-DRO at 166 mg/kg, and no Risk -Based Rule analyses was conducted for this sample. Plans include the removal of soil, followed by over -excavation sampling from the sidewalls and bottom of the excavation (unless groundwater is encountered), and if results are less than the Residential MSCCs, submit an Initial Abatement Action Report. A 24-Hour Report is also planned to be submitted. Questions: 1) Is the proposed actions adequate, too little, or too much? 2) If we encounter groundwater, will the installation and sampling of a monitoring well be required? This may depend on what we see after encountering groundwater (sheen, LNAPL, odors, no odors, etc.). 3) If we do not sample groundwater, would a Notice of Residual Petroleum be required for the NFA?
