FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Discussed that a 6,000 gallon tank may be added and the current 4,000 gallon tank removed from this permit (which will keep the volume under 25,000 gallons/high capacity). This permit modification will require a SDTF application (revised to reflect the actual tanks to be covered under the new permit) and a pre-operational inspection on the 6,000 gallon storage tank. Gave Ms. Locklear a SDTF application form. The placement of the 6,000 gallon tank shall be at least 50 feet from the neighboring property line. Once the 4,000 gallon tank is removed from the permit it can stay on the site but remain empty. 2. The currently permitted tanks (one 4,000 gallon & two 8,000 gallon) appear structurally sound. 3. Noted a small septage spill on the ground behind the yellow 8,000 gallon tank, but the area was cleaned up while on site. Otherwise it appears that septage is being transferred to/from the facility in a safe and sanitary manner. No vectors or odors noted. 4. The approved setbacks are being met. No change in facility. 5. The required 5 foot cleared radius (free of excess vegetation or debris) is present around the tanks at this time. 6. The required signs are posted and the access road is adequate.
