The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Submittal under review for the permit renewal of the two current tanks. The two tanks appear structurally
sound. The required 5 foot cleared radius (free of excess vegetation or debris) is present around the tanks at this
time. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No spills or
leaks noted. The approved setbacks are being met. The required signs are posted and the access road is
adequate. No apparent vectors or odors noted. Requested a SDTF application form with a current signature
& date (the recently submitted application had a signature date of October 2018).
2. If these permitted storage tanks are used by a firm other than Belton’s Septic Tank Service then a septage
detention log shall be maintained to record all discharges into the storage tanks. Enclosed is an example log.
3. A maximizer and an additional storage tank are on site but not permitted for use at this time. The
maximizer has not been used and the storage tank sounds empty. If this facility is to become high capacity (in
excess of 25,000 gallons) then the tanks shall be located at least 200 feet from the creek to the west – normal
setback would be 100 feet from the creek. The distance from the creek to the current location of this new tank
was not measured at this time.
