The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Once a year on site meeting. Reviewed site logs, it appears that the required information is being maintained.
Mr. Carter plans to attend the December class in Raleigh. Soil samples will be taken in the fall of 2020 by the
site operator.
2. The winter cover crop has been planted. A sufficient cover crop is required for land application to occur this
winter. Continue efforts next spring to improve the Bermuda & control weeds/other vegetation.
3. The field markers appear adequate – missing posts noted on the last audit have been replaced. The two corner
posts on the south end of the site are partly hidden by vegetation and Mr. Carter stated that he would remove the
excess vegetation from around the posts.
4. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are being
met but were not measured at this time. No sign of heavy or recent application. No vectors or odors noted.
5. The required signs are posted and the access road is adequate.
