STATUS OF PERMIT:
Permit was set to expire on April 21, 2019. An extension was granted for 1 year (FID 1318915) which the PTO to expire April 21, 2020. PURPOSE OF SITE VISIT: Facility New Cell Construction Inspection STATUS OF PAST NOTED VIOLATIONS: None

None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 – The reason for this inspection was to check on the construction & finalization of New Cell #14.
02 – Matt Einismann, Michael Kavanaugh (Republic Services) and John College (DWM) met at this facility and called Ethan Caldwell (DWM) to discuss areas to be inspected for final review to permit this new cell. 03 – Inspected the power to the pumps. Power is on. 04 – Rain Flaps (3) are constructed and appear to be working fine. They are also welded to the liner. 05 – The outer berm around the cell has 3:1 side slopes and has been seeded and has hay with matting in place to hold the hay in place to prevent erosion. 06 – There is silt fencing installed 20 feet past the toe of the side slopes to prevent silt from leaving this site. 07 – Edge of liner (10 feet from anchor trench) and edge of cell markers are in place. 08 – The gravel in the leachate drain pipe area was viewed and appears to be correct. 09 – It appears this cell is ready to receive waste upon approval from the permitting section. 10 – Digital pictures were taken and some attached to this report.
