The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. This SLAS permit will no longer be tied to L & L Environmental Services (the business was recently sold).
Gave Mr. Oaks a nonpumper form to be submitted to the Raleigh Central Office (by December 31st) with $400
under the new name of Color Purple Farm. Discussed the nonpumper status versus a permitted firm.
2. Mr. Oaks stated that at this time he wished to hold on to this SLAS permit (currently no septage activity). Gave
Mr. Oaks SDTF & SLAS authorization forms which would be required if in the future he plans to allow trucks
from WRE/L&L Environmental onto the farm to discharge. Reviewed the permit submittal and made revisions
as needed. Discusssed renewing the SLAS permit (under the new name of Color Purple Farm) for the two
current fields with field 1 inactive (fescue not re-established).
3. Raining during time of inspection – did not drive over entire site. Fescue appears adequate on Field 2. Field
markers are still missing at the swale on Field 2 but no land application has occurred in a while. Before any
septage is discharged on Field 2 the required field markers will be posted at the swale. It appears that the
approved setbacks are being met and no sign of land application outside the permitted boundaries. No sign of
heavy or recent application. The required signs are posted and the access road is adequate (but was muddy in
some areas).
4. Please note that 3 hours of continuing education hours will be required each year for this SLAS permit
(even if no septage activity is occurring).
