FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. This SDTF permit will no longer be tied to L & L Environmental Services (the business was recently sold). Gave Mr. Oaks a nonpumper form to be submitted to the Raleigh Central office (by December 31st) with $400 under the new name of Color Purple Farm. Discussed the nonpumper status versus a permitted firm. 2. Mr. Oaks stated that at this time he wished to hold on to this SDTF permit (currently no septage activity). Enclosed is a copy of the last SDTF application for this location – please list the new name (Color Purple Farm) at the top and put a new signature & date at the bottom of the second page so that a new permit may be issued for the name change. 3. The dewatering box is at the Charlotte SDTF facility which was part of the sale of L & L Environmental Services and a new SDTF permit will be issued under that new company name for that location. 4. It was raining during the time of this inspection. The three septage tanks (#1, #2 & #3) appear structurally sound and Mr. Oaks stated that they had been emptied prior to the company sale. No leaks/ drips noted. It appears that septage was transferred to/from the facility in safe and sanitary manner. No vectors or odors noted. The approved setbacks are being met. The required 5 foot cleared radius is present around the tanks at this time. The wheelbarrow at the unloading area had been emptied of screening debris. The required signs are posted and access road is adequate. Please contact me if you have any questions or concerns regarding this inspection report. _ Phone: (910) 433-3352 Connie S.
