The soil excavation and hydraulic lift removal activities consisted of the following tasks: 1) Health and Safety Plan preparation; 2) Site Plan review and private utility mark out; 3) soil excavation and lift removal; and 4) reporting. Antea USA of North Carolina, Inc. (Antes USA of NC) prepared a site -specific Health and Safety Plan (HASP) for the specified work. The HASP identified pertinent health and safety concerns for the site, the scope of work and necessary safety pre -cautions as well as response processes for this project. The HASP was reviewed by all field team members, including subcontractors prior to the start of the work. All available site plans and figures were reviewed by Antea USA of NC to determine the locations of known underground utilities, verify equipment access, and, identify the work area. A public utility locate, North Carolina 811, was requested for the site and a private utility locator, Ground Penetrating Radar Systems (GPRS), was contracted to scan for in an attempt to identify subsurface utilities within the work area. A subject property location map is indicated on Figure 1. SECTION 2.0: HYDRAULIC LIFT REMOVAL AND EXCAVATION SECTION 2.1: EXCAVATION METHODOLOGIES Soil excavation and lift removal activities were initiated on August 7, 2019. Hazmat Emergency Response & Remediation, Inc. (HERR) was contracted to remove the existing underground lift under the supervision of Antea USA of NC personnel. The work was performed within service bay #7 as illustrated on Figure 2. The compressed air supply piping for this hydraulic lift was isolated and the lockout was verified by Antea USA of NC. No floor drains were present near the location of the hydraulic lift. Following subsurface utilities being cleared by GPRS, the concrete slab was saw cut to approximately 7.5-feet by 12-feet. Once cut, the concrete floor capping the proposed excavation area was broken into smaller pieces and removed for subsequent off -site disposal by HERR. Following removal of the concrete floor, the exposed compressed air line feeding the lift was disconnected and a vacuum truck was utilized to extract any remaining hydraulic oil from the line. Once accessible hydraulic oil had been removed, the compressed air line was plugged at the end closest to the hydraulic lift to prevent any potential discharge and to keep soil from clogging the lines. HERR began excavating soil from the southern side of the lift and former hydraulic fluid tank using a mini -excavator. The hydraulic lift consisted of two steel hydraulic pistons approximately seven (7) feet in height with an approximate 5-foot high fiberglass hydraulic oil reservoir between the two pistons. The hydraulic pistons and oil reservoir were mounted on a steel bracket anchored to a subsurface concrete slab. No broken parts, cracks, or defects on the lift were observed during the inspection. Once removed, staining was observed on soils directly beneath the bottom of the hydraulic oil reservoir. No staining was visible on the surface of the concrete slab the lift was anchored to. Photographs of the lift removal activities are included in Appendix A. Residual oil remaining in the lift was extracted via HERR vacuum truck. Approximately 39 gallons of hydraulic oil and contact water from inside the oil reservoir and piston housings were transported to HERR's Whitesville, North Carolina facility for treatment and disposal. The hydraulic lift was transported off -site for recycling at Asheboro Recycling in Asheboro, North Carolina. The hydraulic oil and hydraulic lift transport manifests have been included in Appendix B. Following removal of the lift, soil samples were collected from the sidewalls of the excavation and screened for volatile organic compounds utilizing a RAE Systems MiniRae3000 photoionization detector photoionization detector (PID). Due to elevated screening concentrations, the size of the excavation was increased to approximately nine feet wide (east -west) by twelve feet long (north -south) and to a depth of eight feet below ground surface (bgs). Soil consisted primarily of sand and clay fill and was excavated using a mini -excavator. Pneumatic lines were encountered along the northern wall of the excavation associated with the former lift operation. As previously referenced, a concrete slab was encountered at approximately eight feet bgs. The concrete slab had no apparent cracks or seams and was approximately four feet by six feet and centered in the base of the excavation. On August 8, 2018, a total of six post excavation soil samples were collected from approximately eight (8) feet bgs. Two samples were collected along each of the longer sidewalls (Bay 7 — Sidewall East -1 and Bay 7 — Sidewall East -2) and 1 sample was collected from each of the shorter side walls (Bay 7 — Sidewall North and Bay 7 — Sidewall South) (Figure 2). The samples were field screened utilizing a PID. PID readings indicated the potential for volatile organic compounds (VOCs) or semi -volatile organic compounds (SVOCs) to be present in some of the samples collected. In addition, no soil staining was observed from along the sidewalls or base of the excavation. All soil samples (8 total) were placed into laboratory provided containers and submitted for laboratory analysis to Test America, Inc. of Pensacola, Florida. The soil samples were analyzed for Total Petroleum Hydrocarbons (TPH) Diesel Range Organics (DRO) by Environmental Protection Agency (EPA) Method 8015C. In addition, samples were collected for analyses by Extractable Petroleum Hydrocarbons (EPH) by Massachusetts Department of Environmental Protection (MADEP) method and SVOCs by EPA Method 8270D. The laboratory was instructed to hold the EPH and SVOC analyses pending receipt of the laboratory analytical results of the TPH DRO analysis. Photographs of the lift removal activities and soil excavation are included in Appendix A. Excavated soil was stockpiled in two covered polyethylene lined roll -off bins, provided by HERR, in the northeast and southeast corners of the property for subsequent off -site disposal. Following the completion of excavation activities on August 8, 2019, the excavation was backfilled with approximately 33.24 tons of Number-57 (#57) stone to the area just underneath the base of the surrounding concrete floor covering. On August 13, 2019, Antea USA of NC personnel returned to the site to oversee the replacement of concrete by Allen's Concrete. The floor was completed with approximately 8-inches of 4,000PSI concrete and leveled to match the surrounding concrete floor. The roll -offs containing stockpiled soil were subsequently divided into three loads to comply with DOT weight restrictions and approximately 34.58 tons of soil were transported for off -site disposal to AES of NC in Sanford, NC on August 28, 2019 and September 3, 2019. XINOGEN® us.anteagroup.com The logo and ANTEA are registration trademarks of Antea USA, Inc. 2 0 0 antea`group SECTION 2.2: SOIL ANALYTICAL RESULTS SUMMARY AND EVALUATION The laboratory analytical results from the post -excavation soil samples collected on August 8, 2019 are summarized in Table 1 and the laboratory analytical results and chain of custody records are provided in Appendix C. Soil sampling locations relative to the area of excavation are illustrated on Figure 2. TPH DRO was detected in samples BAY 7 — Sidewall East -2 (3,360 milligrams per kilogram [mg/kg]) and BAY 7 — Sidewall West -2 (3,890 mg/kg) at concentrations exceeding the North Carolina TPH Action Level of 100 mg/kg. As a result, the laboratory was directed to analyze the samples for EPH and SVOC analyses. The results of the additional analyses identified bis (2-ethylhexyl) phthalate, C9-C18 aliphatics, C19-C36 aliphatics, and C09-C22 aromatics at concentrations exceeding laboratory method detection limits. Only C09-C22 aromatics in samples BAY 7 — Sidewall East -2 (179 mg/kg [F1, F2]1) and BAY 7 — Sidewall West -2 (2,300 rill were detected at concentrations exceeding the respective soil -to -groundwater standard of 31 mg/kg. SECTION 3.0: PROJECT SCOPE SUMMARY AND CONCLUSIONS The completed scope included hydraulic lift removal and associated soil excavation, confirmation soil sampling of the excavation, laboratory analytical services of the confirmatory soil samples and reporting. Observations made during lift removal activities, the lift appeared to be intact with no visible damage. The concrete slab which was encountered at eight (8) feet bgs did not contain any cracks or seams, and there was no indication of a suspected release of petroleum (e.g. staining). The concrete slab at the base of the excavation was at least 1.5 feet in thickness and was not indicated on design drawings provided to Antea USA of NC. Following excavation activities conducted August 7-8, 2019, a total of 34.58-tons of non -hazardous soil and concrete debris were transported off -site for disposal. Confirmatory soil samples were collected from the sidewalls of the excavation. Laboratory analysis of soil samples collected indicated the presence TPH DRO in samples BAY — 7 Sidewall East -2 and BAY 7 — Sidewall West — 2 at concentrations exceeding the TPH DRO Action Level. As a result, the laboratory was directed to run the samples for EPH and SVOC analyses. Antea USA of NC concludes the following: • Only C09-C22 aromatics in samples BAY 7 — Sidewall East -2 (179 mg/kg [F1, F2] ) and BAY 7 — Sidewall West -2 (2,300 mg/kg) were detected at concentrations exceeding its respective soil -to -groundwater standard of 31 mg/kg. • The former location of the hydraulic lift falls within the existing building footprint and is capped by an approximate 8-inch concrete floor. • The site is currently a commercially developed property and is likely to remain a commercial property in the foreseeable future. 1 F1 qualifier indicates that the matrix spike and/or the matrix spike duplicate recovery is outside the acceptance limits.
