PURPOSE OF SITE VISIT:

STATUS OF PAST NOTED VIOLATIONS:
The damaged sign has been replaced.
OBSERVED VIOLATIONS:
1. Upon review of the land application site logs it was determined that records for the discharge events performed
by Hunt Septic Tank Cleaning at this site in 2019 were not available as per Rule .0842(e)(1 &2). And also that
copies of the land application site logs had not been submitted regularly to the Division as per Rule .0842(e)(3).
Submit to this office (by November 20th) a copy of all the land application site logs for this site, for all
discharge events performed by Cooper Septic Tank Service and Hunt Septic Tank Cleaning in 2019.
Beginning in 2020 submit a copy of the logs regularly (as long as any firm other than Cooper Septic Tank
Service is using the site). The review of the land application site logs that were available (during this inspection)
indicate that the required information is being maintained.

ADDITIONAL COMMENTS
1. Discussed the inspection procedure with Ms. Cooper. Reviewed the site map & field markers with Ms. Cooper.
It appears that the broken posts noted on the last audit of 4/10/2019 have been replaced. However, the missing
marker at the front corner of the site (130 feet from the centerline of the road & 500 feet from the trailer)
is still missing and needs to be replaced. Also some excess vegetation should be removed from around some
field posts.
2. The winter rye has started to come up on the two fields but is not well established yet. A good cover crop is
needed for land application to occur this winter. Continue efforts next year to improve the Bermuda & control
weeds/other vegetation.
3. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are being
met but were not measured at this time.
4. The required signs are posted and the access road is adequate.
5. Noted spray paths on Field 2 at this time. No sign of heavy application. No odors noted.
6. Soil samples shall be taken by the permit holder in the fall of 2020.

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
