OBSERVED VIOLATIONS:
1. Inspected vehicle tag#YA126977, VIN#1HS4JAZN0NH460859, 2300 gallons. Outlet covers were missing –
replace the covers per Rule .0835(a)(3). The “LLC” in the firm name was missing on the passenger side of the
vehicle – replace the “LLC” per Rule .0835(b). Otherwise the lettering was correct on both sides of this
vehicle, but some decals were slightly damaged. The tank appears structurally sound & securely attached. No
drips/leaks were noted.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#YA151104, VIN#1HSHJAZN1PH514950, 2500 gallons. The tank appears structurally
sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both
sides of the vehicle per the information on the firm application. However, some decals were slightly damaged.
2. Logs were reviewed at this time and it appears that the required information is being maintained (including
when the storage tank is used at SDTF#83-04).
3. Submit the firm renewal application by December 31st to avoid the late fee.
