The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Reviewed logs – it appears that the required information for the truck pumping logs is being maintained. Mr.
Locklear attended the 2019 training class in Shallotte. Ensure that the firm permit renewal is submitted by
December 31, 2019 to avoid the late fee.
2. Inspected two trucks – tag# YA60197 & tag#YA022317, VIN# correct per the firm application. The tanks
appear structurally sound & securely attached on both vehicles. The lettering is correct on both sides of both
vehicles at this time (as per the information on the firm application). However, the lettering is slightly damaged
on both vehicles & will need repaired sometime in the future. Outlet covers were on both vehicles and no
leaks/drips were noted.
3. The third vehicle listed on the firm application (tag#YA58158) is not in use.
4. Mr. Locklear stated that the tank currently on vehicle tag#YA60197 will be switched to another truck. When
this occurs provide this office with the correct tag & VIN numbers plus photos showing that the tank is
securely attached to the new vehicle. It is my understanding that switching the tank will not affect the lettering
currently in compliance.
