The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected four vehicles in regards to the firm permit. Inspected the following vehicles.
Tag#FE-5135, VIN# correct per the firm application.
Tag#HA-1161, VIN# correct per the firm application.
Tag#JV-9510, VIN#3C7WRTCL0GG251925, 300 gallons - this vehicle not listed on the firm application.
Tag#J5-1013, VIN#2NKHHM6X2LM386031, 1100 gallons – new vehicle/ not listed on the firm application
The tanks on all four vehicles appear structurally sound & securely attached. The outlet covers were on all four
vehicles and no drips/leaks were noted. The lettering is correct on both sides of the four vehicles per
information on the submitted application – however the phone number displayed on the vehicles was 910-226-
2000, “Inc” is not part of the firm name on the vehicles and the town name is displayed as part of the firm
name.
2. Reviewed the truck pumping logs – it appears that the required information is being maintained.
3. Ensure that the application submitted this winter for the firm permit renewal lists all four vehicles and
the correct tag & VIN for each, plus the correct phone number & contact info for the firm.
