1) Inspected pumper vehicle with License Tag # YA151048. The permit number, town name, and phone
number as noted on the application were present on both sides of the pump truck. All letters were
greater than three (3) inches in height. While the firm name as noted on the application was present on
both sides of the vehicle, all letters of the name did not meet the required height. However, “Fast Lane
Septic” was on both sides of the vehicle and each letter was greater than three (3) inches in height. I
gave Chris the option as to go with “Fast Lane Septic” or update the lettering for the firm name as noted
on the application so that each letter would be three (3) inches or greater in height. Chris chose to
update the lettering for the firm name as is noted on the application. Chris sent me pictures on July 7th
and 8th showing the updated lettering. Base on my observations on 07/02/2019 and Chris’s pictures, the
lettering meets the requirements as stated within the Septage Rules. See pictures below.
2) The steel tank appeared to be structurally sound.
3) Tank bolted to the frame of the truck.
4) Access ports appeared to be in working order and properly sealed.
5) Valve appeared to be in working order. Cap present on the port.
6) No leaks noticed.
7) Permit was issued on 07/12/2019 and has an expiration date of 12/31/2019.
