STATUS OF PERMIT: Active Issued: October 31, 2018 Expires: September 2, 2021 PURPOSE OF SITE VISIT: Routine Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Currently, the Septage Land Application Site consists of a 17.7-acre site divided into Field 1 (7.6 acres), Field 2 (2.0 Acres), Field 3a (6.3 acres) and Field 3b (1.8 acres). Field 3a and 3b shall be managed as one field in accordance to the nutrient management plan and all the fields shall remain in fescue. Field 2 2. The site is permitted for land application of domestic septage (including portable toilet waste) and grease trap pumpings. 3. The site has accessible all-weather roads to the septage land application fields covered under the permit. 4. All fields had a good crop of fescue with not heavy septage land application observed. Border markers were in place. 5. Field 1 was in the 30 day waiting period before harvest and appeared to be in good condition. According to records, the last land application was on 8/30/2019. 6. Field 2 (see photo above) was in use and in good condition. One boundary marker needs to be replaced. 7. Fields 3a and 3b were observed in good condition with cattle. Both fields are currently being managed as one field. According to records, the last land application was on 7/23/2019. The company currently has plans to manage these fields separately in the future; however, this will require a permit modification. 8. Land application records were reviewed and appeared to be in order.
