OBSERVED VIOLATIONS:
1. .0842(a)(17) Approved nutrient management plans shall be followed. Permit Condition 2. This site shall be
operated and maintained in accordance with the nutrient management plan submitted by Randy & Aaron Cheek
and approved by the Division of Waste Management. The fescue is very tall on Field 2 as a result of not being
grazed or cut in a timely fashion. The fescue is not so tall on Field 3. There is a severe weed problem on Fields 2
& 3. Contact your regional agronomist or extension agent for a site visit (& written evaluation to provide to
this office) to evaluate the fescue & for advice on the proper weed control for this particular weed problem.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The two corner field markers on field 3 (along the tree line) are leaning – straighten these posts. Also the
neighboring vegetation is encroaching on the corner field marker on the far back corner of field 3 – cut back the
neighboring vegetation.
2. It appears that the approved setbacks are being met. No sign of land application outside the permitted
boundaries.
3. No apparent sign of heavy or recent application. No odors noted.
4. The required signs are posted and the access road is adequate.
5. No soil sampling required this fall. Soils samples to be taken by the operator in the fall of 2020.
6. Randy & Aaron Cheek went to the Hickory training class in January 2019.
