PURPOSE OF SITE VISIT: Follow-up STATUS OF PAST NOTED VIOLATIONS:
On 09/03/19 inspection, an observed violation to Rule .0835 (b) was noted for not all required information
on both sides of the pumper vehicles and for the letters being less than three (3) inches in height. All
required information was observed on both sides of the vehicles and the letters were observed to be three
(3) inches or greater in height.

OBSERVED VIOLATIONS:
None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

Trucks with VIN# of 1FDWF37P96EC09807, 1FDUF5GT3CKB24649, and 1FDUF5GY3FED71914 are no
longer in use.
2) Follow-up inspection conducted to confirm that the correct lettering and letter height was present on
both sides of the pumper vehicles. All pumper vehicles listed above had the required lettering on both
sides of the vehicles and all letters were three (3) inches or greater in height. The lettering was partially
blocked on two of the vehicles by hoses on the tanks. Please make sure that all required lettering is not
obstructed from view.
3) The tanks on all six (6) trucks were made of metal and permanently affixed to the frames of the trucks.
4) Access ports appeared to be in working order and properly sealed.
5) Valve appeared to be in working order. Caps present on the hose ports.
6) No leaks noticed.
