On March 16, 2000, the Underground Storage Tank (UST) Section of the Division of Waste
Management (DWM) at the Mooresville Regional Office received a Preliminary Site Assessment (PSA)
report for the subject site. According to the PSA, there is at least one known UST present on site. In
order to help the DWM determine ownership and former use of the on -site UST, please answer the
following questions on the attached affidavit. Please be as specific as possible and attach any additional
information necessary to substantiate your responses.
1. When and from whom did you purchase or inherit the property?
2. Do you currently own the property? If not, who owns the property?
3. What were the past uses of the property?
4. When was the UST installed and who installed it?
5. Did you ever own or operate the UST?
6. When was the UST last used and who was the last person to operate the UST?
7. What product was stored in the UST and what was the use of the UST?
8. Who do you believe owned the UST?
9. Include any other information that you think is relevant regarding the UST.
Please provide a current phone number and mailing address for each individual that is mentioned
in your affidavit. Please submit the attached affidavit within 30 days of receipt of this letter. The
